Job Details
An Information and Assessment Officer is required to support Adult Social Care services, working closely with Social Workers and other professionals to deliver effective, customer-focused support to Hackney residents and their carers. JD available on request.
- Work alongside Social Workers and other Adult Social Care professionals to provide accessible, high-quality information and initial conversations.
- Deliver a high standard of customer care through the provision of information, advice and assistance regarding council services and support available from statutory, independent and voluntary agencies.
- Undertake initial conversations using a strengths-based approach to identify needs and promote independence.
- Act as a Trusted Assessor, providing simple equipment to older and disabled people to help them maintain independence.
- Produce individually tailored plans for simple equipment provision.
- Recommend and requisition Telecare solutions where appropriate.
- Support the setup of social care support in line with assessed needs.
