Senior Physiotherapist – Discharge to Assess

Tripod Partners are recruiting for a Senior Physiotherapist to support the Discharge to Assess function within Bolton Council’s Independent Living Service. This locum opportunity will focus on facilitating timely hospital discharges, supporting community follow-up and helping adults maintain their independence within their own homes.

The Independent Living Service works alongside Community Therapy and Neighbourhood teams and includes Occupational Therapists, a Disability Officer, Community Assessment Officers, the Sensory Team and Telecare Team. You will contribute to an integrated approach to care, using strengths-based practice to promote independence, wellbeing and positive outcomes for individuals and carers.

The role involves specialist assessments, person-centred support planning, risk assessments, equipment provision and moving and handling advice. You will work collaboratively with health and social care professionals to reduce the risk of hospital readmission and connect people with appropriate reablement, universal and voluntary services.

Key Responsibilities

• Provide specialist needs assessments for people with disabilities and complex physiotherapy needs.
• Support timely hospital discharges through the Discharge to Assess function and provide appropriate community follow-up.
• Complete Care Act 2014 assessments, including support planning and risk assessments.
• Develop individual, person-centred care and support plans that promote independence through physiotherapy advice, techniques, equipment, adaptations and appropriate support.
• Manage a clinical caseload of service users with complex needs, applying professional judgement to determine appropriate interventions.
• Act as a trusted assessor for equipment provision and moving and handling support.
• Provide specialist advice and guidance to service users, carers and care providers on the nature, impact and management of disability.
• Deliver training to care staff and informal carers on the safe use of equipment and appropriate care procedures.
• Contribute to recommendations for major and complex adaptations, liaising with housing services where necessary.
• Work collaboratively with Community Therapy, Social Work and wider multidisciplinary teams to coordinate care and support.
• Recommend appropriate services and interventions to healthcare professionals and partner agencies to meet individual needs.
• Promote independence through links with reablement, universal services and voluntary organisations.
• Contribute to the development of innovative approaches across health and social care to improve the experience of individuals and carers.
• Support approaches that reduce the risk of hospital readmission and promote wellbeing within the community.

About You

We are looking for an experienced Senior Physiotherapist who can confidently manage a complex caseload and contribute to effective hospital discharge and community-based support. You will have strong assessment skills and be able to develop person-centred plans that promote independence and address individual risks and support needs.

You will be comfortable working within an integrated health and social care environment, collaborating with Community Therapy, Social Work and other professionals. Experience with equipment provision, moving and handling, disability assessments and supporting people to remain safely at home will be particularly relevant to this role.

A proactive, strengths-based approach is essential, alongside the ability to provide clear professional advice, support carers and contribute to the ongoing development of integrated services.

Requirements

• Qualified Physiotherapist with relevant professional experience.
• Experience undertaking specialist assessments for people with disabilities and complex needs.
• Ability to manage a clinical caseload independently.
• Knowledge and experience of person-centred care planning and risk assessments.
• Understanding of the Care Act 2014 and its application within Adult Social Care.
• Experience supporting hospital discharge and community-based rehabilitation or follow-up.
• Knowledge of equipment provision and moving and handling assessments.
• Ability to provide advice and training to service users, carers and care providers.
• Experience working collaboratively within multidisciplinary health and social care teams.
• Ability to identify appropriate reablement and community support options to promote independence.
• Strong communication, assessment and professional decision-making skills.
