Job details: URGENT THERAPY TEAM. Physiotherapist or Occupational Therapist required. Working core hours Monday - Friday to support working within The Transfer Of Care Hub. Triage of referrals, MDT discussions regarding discharges, reviewing referrals and identifying suitable discharge pathways. Understanding of coordination and triage experience. DBS required. Working hours: 09.00 -17.00.
