Physiotherapist required for the Community Rehabilitation Team - assessing and providing intervention for a wide range of conditions including falls and orthopaedics, The caseload will include patients with complex health and social care issues. The post holder will also be involved in supervision of junior staff, students and service improvement. The Community Rehabilitation Teams within East Kent are multi-disciplinary teams that include Occupational Therapists, Physiotherapists, Speech and Language Therapists, Specialist Nurses and Therapy Assistants. The team work within patients’ homes, rehabilitation groups, Community Hospitals and other care settings, to ensure effective discharge planning and a seamless rehabilitation pathway. they provide holistic assessment of patients’ needs and help patients achieve maximum independence in daily living activities and improved well-being. The team provide short-term rehabilitation to enable patients, where possible, to remain in the community rather than be admitted to hospital or long-term care, and to live as independently as possible. They have close links with Acute Hospitals to ensure patients are able to continue their rehabilitation in the community following discharge home. The service treats adults with a wide range of physical disabilities. Skills/Experience: Essential Degree in physiotherapy and minimum of 3 HCPC Registration Evidence of relevant, broad-based clinical experience to include; Care of the older person, musculoskeletal Experience of working with assistants/support workers Evidence of multidisciplinary working Understanding of clinical governance and its implication Knowledge of recent government initiatives and their impact on practice Evidence of relevant Continuing Professional Development, within the last two years Adaptable communication skills, demonstrating a wide range of skills used appropriately in complex situations. Well developed autonomous and team approach to prioritisation and organisational skills. Ability to work autonomously, independently and as part of a team. Accurate and legible note taking.
