Community Physiotherapist – Band 6

Based at Liskeard Community Hospital, part of Cornwall Partnership NHS Foundation Trust, this community rehabilitation role offers the opportunity to deliver high-quality physiotherapy services to patients in their own homes across the local community.
This is an excellent opportunity for a physiotherapist with strong community rehabilitation experience who enjoys autonomous working while collaborating closely with multidisciplinary teams. You will carry out advanced assessments, develop evidence-based treatment plans, and support patients to maximise their independence and functional recovery within their home environments.
Key Responsibilities
- Provide advanced physiotherapy assessment, diagnosis, and treatment for patients with a wide range of complex physical conditions.
- Deliver high-quality rehabilitation programmes within patients’ homes and other community settings.
- Develop individualised treatment plans that promote recovery, independence, and quality of life.
- Manage a varied community caseload, prioritising referrals according to clinical need.
- Travel daily to patients’ homes to deliver assessment and rehabilitation interventions.
- Work collaboratively with multidisciplinary teams, GPs, community services, carers, and families to coordinate patient care.
- Provide leadership, supervision, training, and support to junior physiotherapists and support staff.
- Contribute to the ongoing planning, development, and improvement of community physiotherapy services.
- Maintain accurate and timely clinical documentation in accordance with HCPC and NHS standards.
- Promote safe, effective, and evidence-based physiotherapy practice across the service.
About You
You will be an experienced Physiotherapist with a strong background in community rehabilitation and a passion for delivering person-centred care within patients’ own homes. You will be confident working autonomously, managing a busy community caseload, and building positive relationships with patients and multidisciplinary colleagues.
You will possess excellent assessment, rehabilitation, and communication skills, together with a proactive approach to service development and clinical leadership.
Requirements
- Degree or Diploma in Physiotherapy.
- Current HCPC registration.
- Significant community rehabilitation experience.
- Experience managing complex community caseloads.
- Ability to undertake advanced physiotherapy assessments and develop evidence-based treatment plans.
- Experience supervising or supporting junior staff is desirable.
- Experience using the RIO clinical system is advantageous.
- Full UK driving licence and access to a vehicle are essential.
- Excellent communication, clinical reasoning, and organisational skills.
