Job details:  Community Physiotherapist required for general physical rehabilitation for 4 weeks. Previous experience to include community rehab (physical health), elderly care, acute medicine, orthopaedics.  To provide specialised Physiotherapy assessment, education, advice and treatment to adult patients who have complex health and social care needs; To work as a member of a multidisciplinary team to support hospital discharges, provide rehabilitation in the patients’ home environment and reduce care needs;  To independently manage a defined caseload of complex cases contributing to the provision of a rehabilitation service; To provide a high quality, efficient therapy service to patients with a wide range of physical and medical conditions referred to the Maximising Independence service line. Useful to have UK driving license and access to a car for work and insured for business purpose. Working hours: 37.5 hours per week (flexible)
