Job details:  Integrated Community Services. To carry out comprehensive assessment in patients own home, with the focus being primarily on admission avoidance. Work as part of the MDT to deliver UCR 2hr crisis response including falls pick up service. Skills/experience: Previous experience of community/admission avoidance, proficient in equipment provision and complex manual handling, Car Driver. Working hours: Shifts. Service is open 8 AM to 8 PM, 7 Days per week.
