Job Details:
- Social Care Assistant required (suitable for Occupational Therapy Assistant)
- Assess the needs of adults and older people with physical disabilities, mental health needs and/or learning disabilities, as well as their informal carers
- Undertake assessments in accordance with the Care Act and eligibility criteria
- Work collaboratively with health and social care professionals to support an integrated approach to service delivery
- Develop and implement strengths-based support plans to maximise independence and community living
- Assess activities of daily living and arrange appropriate solutions including minor adaptations, assistive equipment and Assistive Technology
- Screen for major adaptations and support moving and handling equipment needs
- Support service users and carers to identify and commission appropriate resources, including direct payments and private provision
- Provide ongoing assistance to individuals managing rehabilitation and complex support needs
Working Pattern: 2 days in the office, 3 days remote
