Occupational Therapist – Band 6 (5 Roles)

Tripod Partners are recruiting five experienced Band 6 Occupational Therapists for exciting locum opportunities with NELFT, based at Grays Court Community Hospital, providing services across Redbridge, Havering, and Barking & Dagenham. Working within the North East London NHS Intensive Rehabilitation & Therapy Service, you will play a key role in delivering urgent community-based rehabilitation and rapid response interventions that prevent unnecessary hospital admissions and support people to remain safely in their own homes.
This is an excellent opportunity for Occupational Therapists with community or rapid response experience who enjoy working in a fast-paced multidisciplinary environment. You will deliver urgent assessments, provide equipment and rehabilitation interventions, and work collaboratively with health and social care partners to maximise independence and reduce admission risk.
Key Responsibilities
- Provide urgent Occupational Therapy assessments and interventions within patients’ homes to prevent unnecessary hospital admissions.
- Deliver rapid response rehabilitation to stabilise individuals experiencing acute functional decline.
- Assess functional ability, mobility, falls risk, and environmental factors affecting independence.
- Recommend and provide appropriate equipment, assistive technology, and immediate interventions to support safe living at home.
- Develop person-centred rehabilitation plans that maximise independence and reduce the need for hospital admission.
- Support safe discharge pathways and contribute to reducing delays in transfer of care.
- Work collaboratively within a multidisciplinary team to deliver integrated crisis intervention and urgent care planning.
- Liaise with health professionals, Adult Social Care, voluntary organisations, and community services to coordinate ongoing support.
- Monitor patient progress, review outcomes, and adapt treatment plans to support recovery within the community.
- Maintain accurate clinical documentation and contribute to achieving service quality and performance targets.
- Participate in the delivery of a seven-day rehabilitation service across three London boroughs.
About You
You will be a HCPC-registered Occupational Therapist with post-qualification experience within the NHS, ideally gained in community rehabilitation, Intermediate Care, rapid response, or urgent care services. You will be confident working autonomously while collaborating effectively within a multidisciplinary team.
You will possess excellent assessment, clinical reasoning, communication, and organisational skills, together with a proactive and patient-centred approach to supporting individuals with complex rehabilitation needs within the community.
Requirements
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
- Post-qualification Occupational Therapy experience within the NHS.
- Experience within community rehabilitation, rapid response, Intermediate Care, or urgent community services is highly desirable.
- Strong functional assessment, rehabilitation, falls prevention, and equipment provision skills.
- Excellent communication, clinical reasoning, and multidisciplinary team-working skills.
- Full UK driving licence and access to a vehicle are essential due to community working across Redbridge, Havering, and Barking & Dagenham.
- Ability to participate in a seven-day service.
