Reablement Occupational Therapist

 

Brief Job Overview

 

Tripod Partners are recruiting a Reablement Occupational Therapist for a locum opportunity within East Sussex Social Services. Based at Craven Vale, this role supports a 16-bed reablement unit focused on facilitating safe and effective hospital discharges and providing community step-up admissions to prevent unnecessary hospital admissions.

 

The successful candidate will play a key role in supporting individuals to regain independence, maximise functional ability, and achieve positive outcomes through targeted occupational therapy assessment, rehabilitation, and reablement interventions.

 

Key Responsibilities

 

  • Provide occupational therapy assessment and intervention within a reablement unit.
  • Support patients transitioning from hospital to community-based care.
  • Facilitate safe and timely discharges from acute hospital settings.
  • Undertake assessments for community step-up admissions to prevent hospital admission where possible.
  • Develop and implement person-centred reablement plans focused on independence and recovery.
  • Assess functional abilities and identify strategies to maximise daily living skills.
  • Work collaboratively with multidisciplinary teams to coordinate care and rehabilitation goals.
  • Provide advice, guidance, and support to service users, families, and carers.
  • Maintain accurate clinical records and documentation in line with professional standards.
  • Monitor progress and review interventions to ensure positive outcomes are achieved.

 

About You

 

You will be an experienced Occupational Therapist with a strong background in 
reablement, rehabilitation, discharge planning, or community services. You will be confident working with adults who require short-term support to regain independence and avoid unnecessary hospital admissions.

 

You will possess excellent assessment, clinical reasoning, and communication skills, with the ability to work effectively within multidisciplinary teams and manage a varied caseload.

 

Requirements

 

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience within reablement, rehabilitation, community, or intermediate care services.
  • Experience supporting hospital discharge pathways.
  • Strong assessment and goal-setting skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Excellent communication, organisational, and case management abilities.
  • Commitment to person-centred and strengths-based practice.
