Reablement Physiotherapist

Brief Job Overview

Tripod Partners are recruiting an experienced Reablement Physiotherapist for a locum opportunity with the London Borough of Newham. Working within East London Social Services, you will support adults in the community through a strengths-based reablement approach, helping individuals regain independence, improve mobility, and reduce the need for ongoing care following illness, injury, or hospital discharge.

This is an excellent opportunity for a community Physiotherapist who enjoys autonomous working while collaborating closely with Reablement Workers, Occupational Therapists, Social Workers, and wider multidisciplinary teams. The role offers flexible working with a combination of office-based and community visits, delivering person-centred rehabilitation that promotes recovery and prevents unnecessary hospital admissions or long-term care.

Key Responsibilities

  • Undertake comprehensive physiotherapy assessments for adults receiving reablement support within the community.
  • Develop and deliver person-centred rehabilitation programmes that promote independence, recovery, and functional mobility.
  • Assess mobility, balance, transfers, strength, gait, and falls risk to support safe living at home.
  • Work jointly with Reablement Workers, Occupational Therapists, and Social Workers to provide coordinated, multidisciplinary interventions.
  • Undertake community visits independently and alongside other professionals where appropriate.
  • Provide advice, education, and exercise programmes to service users, carers, and families to support self-management and ongoing rehabilitation.
  • Recommend appropriate equipment and interventions to maximise independence and reduce risk.
  • Monitor progress, review rehabilitation goals, and adapt treatment plans to meet changing needs.
  • Maintain accurate clinical documentation and contribute to high-quality service delivery.
  • Support the delivery of timely interventions that reduce hospital admissions and minimise long-term care requirements.

About You

You will be a HCPC-registered Physiotherapist with experience working within community rehabilitation, reablement, intermediate care, or Adult Social Care services. You will be confident working independently within community settings while contributing effectively to multidisciplinary team working.

You will have excellent communication, assessment, rehabilitation, and clinical reasoning skills, together with a strong commitment to delivering person-centred care that enables people to achieve meaningful outcomes.

Requirements

  • Degree or Diploma in Physiotherapy.
  • Current HCPC registration.
  • Experience working within community rehabilitation, reablement, intermediate care, or Adult Social Care.
  • Strong assessment and rehabilitation skills relating to mobility, balance, strength, and functional independence.
  • Experience working collaboratively within multidisciplinary teams.
  • Excellent communication, organisational, and clinical reasoning skills.
  • Ability to work independently within community settings and manage a varied caseload.
  • Commitment to delivering person-centred, strengths-based rehabilitation.
