Tripod Partners are recruiting an Occupational Therapist for a locum role within East Midlands Social Services. This position supports adults within community and discharge pathways, helping individuals maintain independence and safely remain within their own homes.
The Occupational Therapist will deliver a range of assessments and interventions including reablement planning, discharge-to-assess support, equipment provision, moving and handling assessments, and major adaptations. The role offers hybrid working and the opportunity to work within an integrated multidisciplinary environment.
Key Responsibilities
- Undertake holistic and strengths-based occupational therapy assessments.
- Develop personalised goals and reablement plans to support independence.
- Support individuals on discharge-to-assess pathways following hospital discharge.
- Provide community-based therapy interventions to prevent hospital admissions.
- Assess and arrange equipment and home adaptations, including major adaptations where required.
- Carry out complex moving and handling risk assessments and management plans.
- Support service users and carers to actively participate in assessments and care planning.
- Recommend assistive technology solutions and provide demonstrations for safe use.
- Maintain accurate and timely case records and monitor support plan effectiveness.
- Update systems and contribute to service data collection requirements.
- Provide support and guidance to Occupational Therapy Assistants and Social Workers.
- Work collaboratively with health and social care professionals to support seamless transitions of care.
About You
You will be a qualified Occupational Therapist with experience working within
adult social care or adult health services. You will demonstrate strong clinical reasoning
skills and the ability to deliver person-centred interventions focused on reablement and independence.
You will be confident managing a varied caseload independently while working collaboratively within
multidisciplinary teams and fast-paced community environments.
Requirements
- Degree or Diploma in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Experience within adult social care and/or adult health environments.
- Knowledge of safeguarding procedures and the Care Act 2014.
- Experience completing holistic assessments and risk assessments.
- Ability to develop and implement reablement goals and support plans.
- Experience with equipment provision, adaptations, and moving and handling assessments.
- Strong communication, organisational, and partnership working skills.
- Competent IT skills including spreadsheets, databases, and email systems.
