  • Travel daily to patients’ homes to deliver assessment and rehabilitation interventions.
  • Work collaboratively with multidisciplinary teams, GPs, community services, carers, and families to coordinate patient care.
  • Provide leadership, supervision, training, and support to junior physiotherapists and support staff.

  • Promote safe, effective, and evidence-based physiotherapy practice across the service.

About You

You will be an experienced Physiotherapist with a strong background in community rehabilitation and a passion for delivering person-centred care within patients’ own homes. You will be confident working autonomously, managing a busy community caseload, and building positive relationships with patients and multidisciplinary colleagues.
