Tripod Partners are currently recruiting for an experienced Band 6 Physiotherapist to join the Intensive Rehab & Therapy Service at NELFT. This community-based role involves the independent assessment, treatment and discharge planning of patients following their transition from acute hospital care.
The successful Physiotherapist will be confident managing an independent caseload and developing appropriate treatment plans using clinical reasoning and recognised outcome measures. Experience of managing patients who are deteriorating or at risk of deterioration would be particularly desirable, as many patients are discharged into the service from an acute hospital setting.
Key Responsibilities
- Independently assess, treat and support discharge planning for patients within the community rehabilitation team.
- Manage an autonomous caseload and develop individualised physiotherapy treatment plans.
- Use recognised outcome measures and clinical reasoning to guide assessment, treatment and ongoing review.
- Monitor patients’ progress and identify patients who are deteriorating or at risk of deterioration.
- Use acute hospital patient management experience to support safe and effective community rehabilitation.
- Work collaboratively with the wider Intensive Rehab & Therapy Service and multidisciplinary team.
- Support effective discharge planning and rehabilitation within patients’ own environments.
- Maintain accurate and timely clinical records in accordance with HCPC standards.
- Communicate effectively with patients, families and relevant healthcare professionals as required.
About You
We are looking for a confident and clinically experienced Band 6 Physiotherapist who can work autonomously within a busy community rehabilitation service. You will be confident assessing patients, managing your own caseload and adapting treatment plans in response to clinical presentation and progress.
Experience of working with patients in an acute hospital environment would be advantageous, particularly where this has involved recognising and managing patients who are deteriorating or at risk of deterioration. Community rehabilitation experience is preferred, together with the ability to work independently and provide high-quality patient-centred care.
Requirements
- Qualified Physiotherapist with relevant post-qualification experience.
- Previous NHS experience.
- Experience within a community and/or rehabilitation setting is preferred.
- Experience of acute hospital patient management is desirable.
- Confident managing an independent caseload and treatment plans.
- Strong clinical reasoning and assessment skills.
- Ability to use recognised outcome measures to guide treatment and rehabilitation.
- Ability to identify patients who are deteriorating or at risk of deterioration.
- Commitment to maintaining documentation in line with HCPC standards.
- Car driver preferable due to the community-based nature of the role.
Contract & Salary

- Hours: 37.5 hours per week, Monday to Friday
- Working Hours: Shifts, 08:30 – 16:30
- Service Hours: The service currently operates between 08:00 and 18:00, so flexibility should be discussed with management prior to starting.
- Working Pattern: The role starts in the office, with agile working when completing community visits and finishing with patients.
