Tripod Partners are currently recruiting for a Band 6 Physiotherapist to join the Intensive Rehab & Therapy Service at NELFT’s Grays Court Community Hospital. This is an opportunity for an experienced Physiotherapist to work within a community rehabilitation team, providing independent assessment, treatment and discharge planning for patients transitioning from acute hospital care.
The successful Physiotherapist will be confident managing their own caseload, using clinical reasoning and recognised outcome measures to develop and review effective treatment plans. Experience of identifying and managing deteriorating patients would be particularly desirable, given the nature of the service and its role in supporting patients following discharge from acute hospitals.
Key Responsibilities
- Independently assess, treat and support the discharge planning of patients within a community rehabilitation setting.
- Manage an autonomous caseload and plan appropriate physiotherapy interventions based on individual patient needs.
- Use recognised outcome measures and clinical reasoning to guide assessment, treatment planning and review.
- Monitor patients’ progress and recognise signs of deterioration or increased clinical risk.
- Use previous acute hospital experience to support the identification and management of patients who may be deteriorating.
- Work effectively within the Intensive Rehab & Therapy Service and wider multidisciplinary team.
- Ensure patient records and clinical documentation are completed to the required standards and are in line with HCPC requirements.
- Support safe and effective discharge planning and promote rehabilitation within the community.
About You
We are looking for a confident and clinically experienced Band 6 Physiotherapist who is able to work autonomously within a community rehabilitation environment. You will have strong assessment and clinical reasoning skills, be comfortable managing your own caseload and be able to adapt treatment plans in response to changing patient needs.
Experience within an acute hospital setting would be advantageous, particularly where this has involved identifying and responding to patients who are deteriorating or at risk of deterioration. A proactive, patient-centred approach and strong communication skills are essential for this community-based role.
Requirements
- Qualified Physiotherapist with relevant post-qualification experience.
- Previous NHS experience.
- Experience within a community and/or rehabilitation setting is preferred.
- Experience of acute hospital patient management would be advantageous.
- Confident managing an independent caseload and treatment plans.
- Ability to use clinical reasoning and recognised outcome measures effectively.
- Commitment to maintaining accurate clinical documentation in line with HCPC standards.
- Community experience and the ability to work independently.
- Car driver preferable due to the community nature of the role.
