NELFT is seeking an experienced Rehab Assistant – Band 3 to join its Intensive Rehabilitation & Therapy Service. The successful candidate will support patients in their own homes by delivering treatment plans delegated by designated Physiotherapists and Occupational Therapists.

This community-based role involves approximately six home visits per day, making confidence travelling to and working with patients in their own environments essential. The Rehab Assistant will play an important role in supporting rehabilitation programmes, promoting patient progress and ensuring accurate documentation in line with NHS policies and professional standards.

Key Responsibilities

  • Follow and deliver treatment plans delegated by the designated Physiotherapist and Occupational Therapist.
  • Complete approximately six home visits per day as part of the Intensive Rehabilitation & Therapy Service.
  • Provide rehabilitation support to patients within their own homes and community environments.
  • Work confidently with patients in their own environment and support agreed rehabilitation goals.
  • Follow delegated treatment programmes safely and appropriately.
  • Communicate effectively with Physiotherapists, Occupational Therapists and other members of the rehabilitation team.
  • Report relevant patient progress, concerns or changes to the appropriate registered professional.
  • Ensure clinical documentation is completed accurately and in accordance with relevant policies and standards.
  • Work effectively as part of a community rehabilitation and therapy service.

About You

We are looking for a confident and motivated Rehab Assistant with experience working within the NHS, ideally following a post-qualification role within a rehabilitation, community or therapy setting.

You will be comfortable travelling to patients’ homes and delivering delegated rehabilitation programmes independently within agreed parameters. A background in Physiotherapy or Occupational Therapy may also be suitable for candidates looking to gain further NHS experience.

Due to the community-based nature of the role, candidates who can drive are preferable.

Requirements

  • Relevant experience as a Rehabilitation Assistant within the NHS.
  • Post-qualification experience in a rehabilitation, community or therapy setting is desirable.
  • Ability to safely follow treatment plans delegated by Physiotherapists and Occupational Therapists.
  • Confidence working with and treating patients within their own homes.
  • Ability to complete multiple community visits throughout the working day.
  • Good communication and interpersonal skills.
  • Ability to maintain accurate clinical documentation in line with policy and standards.
  • Ability to work effectively as part of a multidisciplinary rehabilitation team.
  • Car driver preferred due to the community-based nature of the role.
  • A background in Physiotherapy or Occupational Therapy may be considered where the candidate is looking to gain NHS experience.
