Occupational Therapist – Reablement Team

Location: Wiltshire Council – Monkton Park Council Offices

About the Role

Tripod Partners are currently recruiting for an experienced Occupational Therapist to join the Reablement Team within Wiltshire Council. This is an opportunity to work within a compassionate and person-centred Adult Community Team supporting people aged 18 and over who are living with physical disabilities, chronic and long-term conditions, autism and sensory impairments.

The role combines specialist Occupational Therapy skills with social care assessment and case management. You will work closely with individuals, families and carers, taking a person-centred, empathetic and adaptable approach to understanding needs, promoting independence and achieving positive outcomes.

You will join an experienced and supportive team where teamwork, wellbeing and mutual respect underpin the service. The role also provides opportunities to contribute to leadership, skills development and best practice within a collaborative and values-led environment.

Key Responsibilities

  • Complete Care Act 2014 assessments with adults aged 18 and over.
  • Identify and assess individuals’ care and support needs.
  • Manage both short-term and long-term cases within the community.
  • Work closely with individuals, families and carers to develop person-centred approaches and outcomes.
  • Apply Occupational Therapy expertise to support independence, wellbeing and participation.
  • Respond appropriately to changing needs and complex circumstances.
  • Progress safeguarding enquiries in accordance with relevant procedures.
  • Support matters relating to the Court of Protection where required.
  • Work collaboratively with health professionals and wider multidisciplinary partners.
  • Contribute Occupational Therapy expertise to multidisciplinary decision-making and care planning.
  • Maintain accurate and appropriate records and case documentation.
  • Contribute to the development of best practice within the team.
  • Work professionally and compassionately while managing a varied caseload.

About You

We are looking for an experienced Occupational Therapist who can combine strong professional skills with a person-centred approach to adult social care.

You will be confident completing Care Act assessments and managing a varied caseload, while also bringing your Occupational Therapy knowledge to the assessment and support of adults with physical disabilities, long-term conditions, autism and sensory impairments.

You will be an effective communicator who can build positive relationships with individuals, families, carers and professional partners. The ability to respond flexibly to changing needs, work collaboratively and demonstrate compassion and integrity is essential.

Requirements

  • Relevant Occupational Therapy qualification.
  • Current HCPC registration.
  • Experience working as an Occupational Therapist within adult social care or a community setting.
  • Experience completing Care Act 2014 assessments.
  • Experience managing both short-term and long-term cases.
  • Understanding of person-centred assessment and support planning.
  • Experience working with adults with physical disabilities and/or chronic and long-term conditions.
  • Ability to work collaboratively with health and multidisciplinary partners.
  • Understanding of safeguarding processes and the ability to progress safeguarding enquiries appropriately.
  • Ability to contribute to or support Court of Protection matters.
  • Strong communication, assessment, organisation and case management skills.
  • Ability to work autonomously while contributing positively to a supportive team.
  • A compassionate, empathetic, adaptable and professional approach.

Working Arrangements

This is a hybrid role, with up to 50% of administrative time able to be completed remotely. Specific working arrangements can be discussed at interview.

The successful Occupational Therapist will be based within the wider service at Monkton Park Council Offices and will work within the community as required.
