Reablement (Care) Worker – 3 Roles

About the Role

Tripod Partners are currently recruiting for 3 experienced Reablement (Care) Workers to join the reablement service within Enfield Council. These roles provide short-term, outcome-focused support to adults in their own homes following illness, hospitalisation or hospital discharge.

The initial placement will be for 12 weeks, with the potential for further 12-week extensions. Workers will generally be allocated to one of four geographical clusters within the borough but must be willing to work across clusters where service requirements demand.

Working alongside Occupational Therapists, Assistant Occupational Therapists and the wider reablement team, you will support individuals to rebuild confidence, regain essential daily living skills and work towards their previous level of independence.

This is an enabling and independence-focused service, rather than a long-term care provision. Support is normally provided for up to six weeks, with a clear focus on achieving positive outcomes for the individual.

Key Responsibilities

  • Provide short-term, person-centred reablement support to adults in their own homes.
  • Support individuals following illness, hospitalisation or hospital discharge.
  • Assist people to rebuild confidence and regain skills in daily living activities.
  • Encourage individuals to maximise their independence and work towards their previous level of functioning.
  • Provide personal care and practical support where required as part of the reablement process.
  • Work collaboratively with Occupational Therapists, Assistant Occupational Therapists and other members of the wider team.
  • Follow agreed reablement plans and contribute to positive, measurable outcomes.
  • Promote an enabling approach rather than creating dependence on care.
  • Work across geographical clusters when required to meet service needs.
  • Provide additional ad hoc cover for annual leave, staff training and sickness absence where available.
  • Maintain professional standards relating to safeguarding, medication, moving and handling and infection prevention and control.

About You

We are looking for experienced Reablement Care Workers who understand the importance of promoting independence and enabling adults to regain confidence and skills following illness or hospital discharge.

You will have experience supporting adults in their own homes and be confident providing personal care and assistance with daily living tasks while maintaining a strong focus on the individual’s abilities and goals.

You must be flexible and able to respond to changing service requirements, including travelling between care calls and working across different geographical clusters within Enfield.

Available Placements

Placement 1 – Cluster 2

  • Shift time: 5:00 pm–9:00 pm
  • Week 1: Monday, Wednesday, Thursday, Saturday and Sunday – 20 hours
  • Week 2: Tuesday, Wednesday and Friday – 12 hours
  • Total: 32 hours over the two-week rolling rota
  • These are the minimum guaranteed rota hours and successful applicants must commit to covering them on an ongoing basis.
  • Additional hours may be offered as they become available.

Placement 2 – Cluster 1

  • Daily split shifts: 7:30 am–11:00 am, 12:00 noon–1:30 pm and 5:00 pm–9:00 pm
  • Unpaid break: 11:00 am–12:00 noon
  • Total working time: 9 hours per day
  • Week 1: Tuesday, Wednesday and Friday – 27 hours
  • Week 2: Monday, Thursday, Saturday and Sunday – 36 hours
  • Total: 63 hours over the two-week rolling rota
  • These are the minimum required hours.
  • The full rota may be allocated to one worker or divided between two suitable workers where candidates cannot commit to all split shifts.
  • Any division of the rota must provide complete and reliable cover.

Requirements

  • At least two years’ experience supporting adults in their own homes.
  • Experience providing personal care and support with daily living tasks.
  • Care Certificate or Level 2 NVQ/QCF qualification in Care.
  • Full UK driving licence.
  • Access to a car for travel between care calls across the borough.
  • Up-to-date training in moving and handling.
  • Up-to-date medication administration training.
  • Up-to-date safeguarding training.
  • Up-to-date infection prevention and control training.
  • Understanding of reablement and an enabling approach focused on independence.
  • Ability to work flexibly in response to changing service requirements.
  • Willingness to work across geographical clusters when required.

Initial Contract: 12 weeks, with likely renewal in further 12-week periods
