Cambridgeshire and Peterborough NHS Foundation Trust is seeking an experienced Neuro Physiotherapist – Band 6 to join the Community Neuro Rehabilitation Service (CNRS) and Early Supported Discharge – Stroke (ESD) service in Cambridgeshire East.

The successful Physiotherapist will work as an autonomous practitioner within an integrated multidisciplinary team, providing specialist assessment and rehabilitation for patients with complex neurological conditions and multiple pathologies. The role will involve working across a range of community settings, including lone working, with access to Advanced Practitioners when required.

Key Responsibilities

• Assess, plan and develop specialist physiotherapy programmes for patients with complex neurological conditions and a wide range of complex needs.
• Work as an autonomous practitioner within the integrated Stroke ESD and Community Neuro Rehabilitation Team.
• Manage an allocated caseload and undertake specialist assessments of patients with complex neurological problems and multiple pathologies.
• Use specialist clinical reasoning skills to assess individual needs and determine appropriate physiotherapy interventions.
• Identify patient needs, agree meaningful goals and provide appropriate therapy interventions addressing physical, functional and cognitive needs.
• Develop and deliver rehabilitation programmes to support patients in achieving their goals and maximising independence.
• Provide adaptive equipment and support self-management where appropriate.
• Refer patients to other services and agencies to ensure their identified needs are met.
• Keep patients at the centre of care and promote patient and carer participation in decision-making.
• Allocate work to junior and non-registered staff.
• Provide support and education to peers, new staff, non-registered staff and students.
• Remain accountable for delegated caseloads held by non-registered staff within the team.
• Prioritise referrals according to clinical need and allocate or signpost appropriately.
• Provide a high standard of physiotherapy care across community settings.
• Work flexibly across various community environments, including lone working, with access to Advanced Practitioners when required.

About You

We are looking for a skilled and motivated Band 6 Neuro Physiotherapist with experience working with patients with complex neurological conditions and multiple pathologies. You will be confident working autonomously and using specialist clinical reasoning to assess patients, establish goals and develop appropriate rehabilitation programmes.

You will have a patient-centred approach and be able to work effectively within an integrated multidisciplinary service. The ability to support and educate junior, non-registered and newly appointed staff will also be important within this role.

Requirements

• Relevant Physiotherapy qualification.
• Current HCPC registration.
• Relevant experience in neurological physiotherapy and rehabilitation.
• Experience assessing and treating patients with complex neurological conditions and multiple pathologies.
• Strong specialist clinical reasoning and autonomous decision-making skills.
• Experience working within a multidisciplinary rehabilitation service.
• Ability to develop and deliver individualised rehabilitation programmes.
• Experience supporting self-management and the use of adaptive equipment.
• Ability to allocate work and provide support and education to junior and non-registered staff.
• Ability to prioritise referrals according to clinical need.
• Ability to work across community settings, including lone working.
