The London Borough of Hackney is seeking an experienced Occupational Therapist to join its Double Handed Care Project. This is an opportunity for an Occupational Therapist to contribute to the assessment and review of care needs, supporting person-centred approaches and appropriate care arrangements within an adult social care setting.

The successful candidate will work as part of the Double Handed Care Project, applying professional Occupational Therapy knowledge to support effective outcomes for service users. The role is offered on a hybrid basis, with a minimum of two days per week required within the service.

Key Responsibilities

• Provide Occupational Therapy input to the Double Handed Care Project.
• Complete appropriate assessments of service users’ functional needs and care requirements.
• Consider how Occupational Therapy interventions, equipment and care arrangements can support independence and safe care.
• Contribute to person-centred care planning and reviews.
• Work collaboratively with colleagues and relevant professionals to support appropriate outcomes for service users.
• Use professional knowledge and clinical reasoning to inform assessment and decision-making.
• Maintain accurate and timely professional records.
• Manage an allocated workload effectively and work autonomously within professional and service requirements.

About You

We are looking for a motivated and experienced Occupational Therapist who is confident working within adult social care and able to apply Occupational Therapy expertise to complex care arrangements. You will have strong assessment and communication skills and be able to work collaboratively with colleagues and other professionals.

You will be committed to person-centred practice and supporting service users to achieve safe and appropriate care arrangements while promoting independence wherever possible.

Requirements

• Relevant Occupational Therapy qualification.
• Current HCPC registration.
• Relevant Occupational Therapy experience, ideally within adult social care.
• Strong assessment and clinical reasoning skills.
• Ability to work autonomously and as part of a multidisciplinary service.
• Excellent communication and organisational skills.
• Ability to work effectively within a hybrid arrangement, with a minimum of 2 days per week as required.
