Occupational Therapist – Adult Hospital & Discharge Team

Job Overview

Tripod Partners are currently recruiting for an experienced Occupational Therapist to join the Adult Hospital and Discharge Team at the London Borough of Haringey. This is an ongoing locum opportunity within North London Social Services, offering flexible and predominantly home-based working.

The role will focus on hospital discharge screening, discharge pathways, admission avoidance, reablement and rehabilitation, functional assessment and equipment provision. The successful Occupational Therapist will work closely with hospital therapy teams and other professionals to determine appropriate discharge pathways and ensure people receive the right support to maximise independence and remain safely at home.

Most screening, equipment ordering and associated paperwork can be completed remotely. Face-to-face assessments will be required within hospitals and occasionally in the community, potentially around one day per week depending on caseload and urgent assessment requirements. There is no routine requirement to attend the council office.

Key Responsibilities

• Complete Occupational Therapy screening for hospital discharge cases.
• Review hospital Occupational Therapy and therapy recommendations and determine the most appropriate discharge pathway.
• Complete functional assessments where further Occupational Therapy input is required.
• Support safe and timely hospital discharge and admission avoidance.
• Order equipment following recommendations made by hospital therapy teams.
• Independently assess and recommend appropriate equipment based on functional need.
• Complete occasional D2A (Discharge to Assess) community visits.
• Undertake hospital-based assessments where functional ability or existing recommendations require further clarification.
• Provide professional Occupational Therapy reasoning and clinical recommendations.
• Professionally challenge or clarify hospital therapy recommendations where the proposed intervention is not considered appropriate based on the individual’s functional needs.
• Work with individuals presenting with complex circumstances, including housing needs, homelessness, mental health needs and wider social care requirements.
• Contribute to reablement and rehabilitation pathways to promote independence and reduce the need for ongoing care and support.
• Cross-cover other Occupational Therapists within the team when required.
• Maintain accurate records and ensure professional recommendations are clearly documented.

About You

We are looking for an experienced and confident Occupational Therapist who is able to work independently and apply strong professional reasoning to complex discharge and social care situations.

You will have a good understanding of functional assessment, hospital discharge, reablement, rehabilitation and equipment provision. You will be confident reviewing recommendations made by hospital therapists and able to explain and evidence your own clinical reasoning when determining the most appropriate pathway for an individual.

The successful candidate will be comfortable working predominantly from home while managing face-to-face hospital and community assessments when required. You should be organised, adaptable and able to prioritise effectively in a busy Adult Social Care discharge environment.

Requirements

• Qualified and registered Occupational Therapist.
• Significant experience within Adult Social Care, hospital discharge or a similar community setting.
• Strong experience of functional assessment.
• Experience of hospital discharge and discharge pathway planning.
• Knowledge and experience of reablement and rehabilitation.
• Experience assessing, prescribing and ordering appropriate equipment.
• Ability to independently determine appropriate equipment based on functional needs.
• Experience undertaking hospital and/or community-based assessments.
• Strong clinical reasoning and professional decision-making skills.
• Confidence to professionally challenge or clarify recommendations made by hospital therapy teams where appropriate.
• Ability to work with complex cases involving housing, homelessness, mental health and wider social care needs.
• Ability to work autonomously and manage competing priorities effectively.
• Strong communication and report-writing skills.
• Willingness and ability to cross-cover other therapists within the team when required.

Working Arrangements

This role offers flexible and hybrid working arrangements, with the majority of screening, equipment ordering and administrative work able to be completed from home.

• Home-based working: Most screening, equipment ordering and paperwork.
• Face-to-face work: Hospital and occasional community assessments.
• Expected face-to-face commitment: Potentially around one day per week, depending on caseload and urgent assessment requirements.
• Office attendance: No routine requirement to attend the council office.
