Reablement Officer

Job Overview

Tripod Partners are currently recruiting for a Reablement Officer to join Enfield Council’s Adult Social Care service. This is an ongoing locum opportunity within North London Social Services, initially offered as a 12-week placement with the potential for further 12-week extensions.

The successful Reablement Officer will undertake home-based assessments, develop goal-focused reablement plans and review individuals’ progress to support people to maximise their independence and achieve their personal goals. The role also includes medication assessments, medication audits, carer competency observations, practice observations and the identification and escalation of risks or safeguarding concerns.

Key Responsibilities

• Complete timely home-based initial assessments and risk assessments.
• Develop person-centred, goal-focused reablement plans providing support for up to six weeks.
• Review reablement plans and monitor individuals’ progress towards agreed outcomes.
• Maintain accurate and up-to-date electronic care records.
• Complete medication assessments, MAR sheets and medication protocols.
• Undertake medication audits and carer competency observations.
• Observe care practice and provide appropriate feedback or escalation where required.
• Identify, manage and escalate risks, safeguarding concerns and other issues requiring management oversight.
• Work collaboratively with individuals, carers, care staff and other professionals to promote independence and wellbeing.
• Undertake borough-wide home visits and lone working safely and effectively.
• Manage a varied workload independently, prioritising assessments, reviews and visits according to need.
• Use approved digital and AI-enabled tools, including Microsoft Copilot and Beam, appropriately and in accordance with professional judgement and information governance requirements.

About You

We are looking for a confident and experienced Reablement Officer with a strong background supporting adults within home or community settings. You will have experience of assessments, risk management, reablement planning and medication management, together with the ability to work independently and safely when undertaking lone home visits.

You will be organised, adaptable and able to respond effectively to changing priorities. Strong written and verbal English skills are essential, alongside confidence using electronic care-record systems and Microsoft Word and Excel.

Experience within reablement or domiciliary care will be particularly relevant to this position. You will also need to demonstrate sound professional judgement when using approved AI and digital tools within a social care environment.

Requirements

• NVQ/QCF Level 3 in Adult Social Care or equivalent qualification.
• At least two years’ experience supporting adults in home or community settings.
• Experience within domiciliary care and/or reablement services is preferred.
• Experience completing assessments and risk assessments.
• Experience developing and reviewing reablement plans.
• Experience of medication management, including medication assessments, MAR sheets and protocols.
• Experience of electronic care-record systems.
• Full UK driving licence and access to a vehicle for borough-wide home visits.
• Strong written and verbal English communication skills.
• Advanced Microsoft Word and Excel skills.
• Confidence using electronic systems and maintaining accurate records.
• Ability to work independently, prioritise effectively and undertake lone working safely.
• Knowledge of approved AI tools such as Microsoft Copilot and Beam, with the ability to use them appropriately and in accordance with information governance requirements.

Working Hours & Rota

The role is offered at 35 hours per week, working from 9:15am to 5:15pm on a four-week rolling rota, including one weekend in four.

• Week 1: Tuesday, Wednesday, Thursday and Friday, 9:15am–5:15pm
• Week 2: Monday to Friday, 9:15am–5:15pm
• Week 3: Monday to Friday, 9:15am–5:15pm
• Week 4: Monday to Friday, plus Saturday and Sunday, 9:15am–5:15pm

Some flexibility is required to respond to emergencies, rota changes or visits that overrun. Where agreed with management, hours may be adjusted on another day.
