Independent Living Assessor
Brief Job Overview
Tripod Partners are currently recruiting for an experienced Independent Living Assessor to join the Hospital Discharge Service at the City of Westminster Council. The team supports adults with complex health and social care needs to return home safely from hospital and remain as independent as possible.
Working as part of a multidisciplinary team alongside Social Work, Occupational Therapy, Nursing and other health and social care professionals, you will play a key role in supporting hospital discharge pathways, coordinating outcome-focused support and promoting independence through proportionate assessment, review and care coordination.
You will work closely with hospitals, health partners, community services, carers and other agencies to ensure residents receive the right support, by the right person, at the right time.
Key Responsibilities
- Screen incoming referrals using a RAG rating process and gather relevant information from wards, discharge teams, Home First services and health partners.
- Review existing packages of care and coordinate outcome-focused interim support plans.
- Attend discharge planning and multidisciplinary meetings, agreeing actions and coordinating next steps.
- Promote the use of assistive technology to maximise independence and reduce reliance on statutory services.
- Process referrals and monitor cases to ensure appropriate services remain in place.
- Complete accurate records, assessments, referrals and reports.
- Liaise and negotiate with internal departments and external agencies to secure appropriate support.
- Provide information, advice and guidance to residents, carers and professionals.
- Support residents to complete self-assessments and make informed choices about care and support options.
- Undertake risk assessments and develop proportionate support plans that balance independence with safety.
- Support personal budget processes and financial assessments.
- Provide information regarding reablement services, financial assessments and discharge arrangements.
- Maintain knowledge of specialist, community and universal services available to residents.
- Manage a caseload of individuals requiring ongoing review and monitoring.
- Work alongside voluntary sector organisations, service providers and community partners to maximise available support opportunities.
- Contribute to service performance monitoring and data collection requirements.
- Undertake annual reviews of support plans using appropriate assessment and review tools.
- Set goals, coordinate support arrangements and review outcomes with residents.
- Promote independence and maximise individuals’ ability to undertake activities of daily living safely.
- Ensure support plans remain responsive to changing needs.
- Work collaboratively with Adult Social Care colleagues and partner organisations to optimise outcomes for service users.
About You
We are looking for an Independent Living Assessor with experience within a health, social care, community support or hospital discharge environment. You will be committed to supporting people to live independently and have a good understanding of care coordination and the Trusted Assessor role.
You will be confident assessing needs, monitoring progress and adapting support plans in response to changing circumstances. Strong communication and relationship-building skills are essential, alongside the ability to work effectively within multidisciplinary teams and build positive relationships with residents, carers and professionals.
You will be organised and able to prioritise competing demands while maintaining accurate records and producing clear, concise reports. A strong commitment to promoting independence, choice and control, alongside equal partnership working with residents and carers, is essential.
Requirements
- Experience working within health, social care, community support or hospital discharge services.
- Knowledge and understanding of care coordination and the Trusted Assessor role.
- Awareness of hospital discharge processes and pathways.
- Understanding of disabilities and long-term conditions and their impact on individuals.
- Knowledge of services available to disabled adults and their carers.
- Understanding of anti-discriminatory practice and the needs of diverse communities.
- Ability to assess needs and develop outcome-focused support plans.
- Ability to undertake risk assessments and balance independence with safety.
- Experience working effectively within multidisciplinary teams.
- Ability to work collaboratively with internal and external partners.
- Ability to manage a caseload and prioritise competing demands.
- Strong written and verbal communication skills.
- Ability to develop positive working relationships with residents, carers and professionals.
