Independent Living Assessor

Brief Job Overview

Tripod Partners are currently recruiting for an experienced Independent Living Assessor to join the City of Westminster Council’s Adult Social Care service. This locum opportunity will involve providing statutory annual social care reviews for older adults and physically disabled adults, ensuring care and support arrangements remain appropriate, person-centred and compliant with the Care Act.

The successful candidate will manage a caseload, complete high-quality reviews both in person and by telephone, undertake any necessary follow-up work and ensure the views of customers, families, friends and other professionals are fully considered. The role is supported by supervision and access to guidance where required, with effective use of case management systems being an important part of the position.

Key Responsibilities

  • Undertake statutory annual social care reviews for older and physically disabled adults.
  • Carry an allocated caseload and complete Care Act-compliant reviews and associated follow-up work.
  • Deliver high-quality reviews both face-to-face and by telephone, depending on individual circumstances.
  • Ensure the voice of the customer is central to the review process, alongside the views of family members, friends and relevant professionals.
  • Gather and consider relevant information to ensure reviews accurately reflect the individual’s circumstances and support needs.
  • Maintain accurate and up-to-date case records using the Mosaic case management system.
  • Make effective use of case management systems and contribute to the continual improvement of the service.
  • Work effectively within organisational policies, procedures and objectives.
  • Undertake any other duties and responsibilities allocated in accordance with the requirements of the service.

About You

We are looking for a confident and experienced Independent Living Assessor with strong communication and assessment skills. You will be comfortable managing a caseload and completing statutory social care reviews for older adults and people with physical disabilities.

You will have excellent written and verbal communication skills and be able to engage effectively with customers, families, friends and professionals. You will be organised, able to manage your workload effectively and committed to delivering high-quality, person-centred reviews.

Requirements

  • Experience undertaking social care reviews within an adult social care setting.
  • Knowledge and understanding of Care Act-compliant reviews.
  • Experience working with older adults and/or physically disabled adults.
  • Ability to manage a caseload and complete associated follow-up work.
  • Excellent written and verbal communication skills.
  • Ability to gather and incorporate the views of customers, families, friends and other professionals.
  • Ability to maintain accurate case records and use case management systems effectively.
  • Commitment to delivering high-quality, person-centred services.
