Job Overview
Hertfordshire County Council is seeking an experienced Occupational Therapist to join its Adult Social Care service, with a specialist focus on major and minor adaptations.
The successful Occupational Therapist will assess the needs of adults with disabilities and long-term conditions, identifying appropriate equipment and adaptations to promote independence, improve safety and enable individuals to remain living within their own homes and communities.
Key Responsibilities

Complete comprehensive Occupational Therapy assessments within a community and social care setting.

Assess individuals for minor and major adaptations based on their functional needs and circumstances.

Identify appropriate equipment, aids and adaptations to promote independence and reduce risks within the home.

Make professional recommendations for major adaptations, including structural alterations where appropriate.

Assess moving and handling needs and provide advice to individuals, families and carers.

Work collaboratively with Social Workers, Housing, health professionals, contractors and other relevant agencies.

Provide clear professional reports, recommendations and specifications for adaptations.

Ensure adaptations and equipment are appropriate, proportionate and person-centred.

Support individuals to maximise independence and maintain their ability to live safely at home.

Manage a caseload effectively, prioritising assessments and referrals according to need and risk.

Maintain accurate case records and ensure assessments and recommendations comply with relevant legislation, policies and procedures.

Provide advice and guidance to service users, carers and professionals regarding equipment and adaptations.

Contribute to safeguarding, risk management and positive outcomes for adults receiving support.
Skills & Experience

Degree or equivalent qualification in Occupational Therapy.

Current HCPC registration.

Previous experience working as an Occupational Therapist within Adult Social Care, community services or a Local Authority setting.

Strong experience assessing and recommending major and minor adaptations.

Experience with equipment provision, moving and handling and home environment assessments.

Good understanding of strength-based and person-centred practice.

Knowledge of relevant adult social care legislation and safeguarding procedures.

Excellent assessment, report-writing and communication skills.
-
Ability to work independently while contributing effectively to a multidisciplinary team.

Strong organisational skills and the ability to manage a varied caseload.
