Brief Job Overview
Tripod Partners are recruiting an experienced Reviewing Officer for a locum opportunity with the London Borough of Hackney. Working within the East London Social Services Access & Duty Team, you will undertake strengths-based reviews and reassessments for people with long-term conditions, helping to ensure that care and support arrangements remain effective, proportionate, and focused on maximising independence.
You will work in accordance with the principles of the Care Act, visiting people in their own homes and care home settings to review support plans, assess changing needs, manage risk, and develop outcome-focused solutions. The role involves autonomous community-based working and close collaboration with service users, families, carers, and wider professionals.
Key Responsibilities
- Undertake strengths-based reviews and reassessments for people with long-term conditions in line with Care Act principles.
- Visit service users in their own homes and care home settings to review needs, outcomes, and existing support arrangements.
- Work directly with service users, families, and carers to review and develop effective support plans.
- Develop appropriate, budgeted, outcome-based support plans using the Resource Allocation System (RAS).
- Monitor established support plans and residential and nursing home placements to ensure services remain effective and promote independence.
- Provide information, advice, and signposting to appropriate universal and community services.
- Assess and recommend Telecare support where appropriate to maximise independence and manage risk.
- Consider the financial implications and costs of support when determining appropriate services to meet assessed needs.
- Identify, manage, and record risks appropriately throughout the assessment and review process.
- Raise and respond to safeguarding concerns in accordance with London safeguarding procedures.
- Accurately capture and maintain qualitative and quantitative information relating to assessments, reviews, outcomes, and service activity.
- Work under the guidance and supervision of the Senior Practitioner while taking responsibility for your own performance and caseload.
- Contribute to achieving relevant performance targets and National Indicator requirements for assessments and reviews.
About You
You will be an experienced Reviewing Officer or Adult Social Care practitioner with strong assessment and review skills. You will have a good understanding of strengths-based practice and the principles of the Care Act, with the ability to develop outcome-focused support plans that promote independence and wellbeing.
You will be confident undertaking lone working in community and care home environments and communicating effectively with service users, families, carers, and partner professionals. Strong risk management, safeguarding, financial awareness, and case recording skills will be essential.
Requirements
- Experience undertaking Adult Social Care assessments, reviews, or reassessments.
- Knowledge and understanding of strengths-based practice and the Care Act.
- Experience developing outcome-based support plans.
- Understanding of Resource Allocation Systems and budgeted support planning.
- Experience working with people with long-term conditions and/or complex care and support needs.
- Experience undertaking community and care home visits, including lone working.
- Strong risk assessment and management skills.
- Knowledge of safeguarding procedures and ability to respond appropriately to safeguarding concerns.
- Ability to provide information, advice, and effective signposting to universal and community services.
- Understanding of Telecare and its role in supporting independence and managing risk.
- Strong written and verbal communication skills, with accurate case recording abilities.
- Ability to manage workload effectively and work towards statutory and performance targets.
