Brief Job Overview
Tripod Partners are recruiting an experienced Band 6 Falls Prevention Therapist (Occupational Therapist or Physiotherapist) for a locum opportunity with Central and North West London NHS Foundation Trust, based at Ealing Day Treatment Centre, Southall. Working within the West London NHS Falls Prevention Service, you will deliver specialist assessments and evidence-based rehabilitation to adults at risk of falls, helping them improve mobility, confidence, safety, and independence within their own homes and the community.
This role is ideal for an Occupational Therapist or Physiotherapist with experience in rehabilitation, frailty, community therapy, or older people’s services who enjoys delivering holistic, patient-centred interventions as part of a supportive multidisciplinary team.
Key Responsibilities
- Provide specialist Occupational Therapy or Physiotherapy assessments for patients referred to the Falls Prevention Service.
- Undertake holistic assessments to identify falls risk, functional limitations, mobility issues, and rehabilitation needs.
- Deliver evidence-based falls prevention interventions through both one-to-one treatment sessions and group programmes.
- Develop personalised rehabilitation programmes that improve balance, strength, mobility, confidence, and independence.
- Monitor patient progress, review outcomes, and adapt treatment plans to achieve the best possible results.
- Provide education, advice, and self-management strategies to patients, carers, and families to reduce future falls risk.
- Work closely with the Lead Occupational Therapist, Lead Physiotherapist, Service Manager, and wider multidisciplinary team to deliver integrated care.
- Support the supervision, training, and development of Rehabilitation Assistants and other team members.
- Maintain accurate clinical documentation in accordance with HCPC standards and Trust policies.
- Participate in clinical supervision, continuing professional development (CPD), audit, and service improvement initiatives.
- Contribute to achieving the Falls Prevention Service objectives through high-quality, evidence-based clinical practice.
About You
You will be a HCPC-registered Occupational Therapist or Physiotherapist with experience working within rehabilitation, falls prevention, community therapy, frailty, or older people’s services. You will have excellent assessment and clinical reasoning skills together with a passion for promoting independence and preventing avoidable falls.
You will be confident managing your own caseload while working collaboratively within a multidisciplinary team and supporting the development of less experienced colleagues.
Requirements
- Degree or Diploma in Occupational Therapy or Physiotherapy.
- Current HCPC registration.
- Experience working within rehabilitation, community therapy, falls prevention, frailty, or older people’s services.
- Strong evidence-based assessment, rehabilitation, and clinical reasoning skills.
- Experience delivering therapy in both individual and group settings.
- Excellent communication, organisational, and multidisciplinary team-working skills.
- Ability to support the supervision and development of Rehabilitation Assistants and junior staff.
- Commitment to person-centred practice, continuing professional development, and service improvement.
