Neuro Rehabilitation Physiotherapist – Band 6

Brief Job Overview

Tripod Partners are recruiting an experienced Band 6 Neuro Rehabilitation Physiotherapist for a locum opportunity with Central and North West London NHS Foundation Trust, based at Ealing Day Treatment Centre, Southall. Working within the West London NHS Stroke and Neuro Service (ENABLE Team), you will provide specialist neurological rehabilitation to adults with acquired and progressive neurological conditions across community outpatient clinics, patients’ homes, and other community settings.

This is an excellent opportunity for a Physiotherapist with experience in neurological rehabilitation who is passionate about delivering evidence-based, goal-orientated rehabilitation that promotes independence, prevents unnecessary hospital admissions, and supports timely discharge following stroke and other neurological conditions.

Key Responsibilities

  • Provide specialist evidence-based assessment, treatment, and rehabilitation for adults with acquired and progressive neurological conditions including stroke, traumatic brain injury, Parkinson’s disease, Multiple Sclerosis (MS), Motor Neurone Disease (MND), and spinal cord pathologies.
  • Manage a varied and complex caseload of patients with neurological, musculoskeletal, orthopaedic, cognitive, and communication impairments.
  • Deliver person-centred, goal-orientated rehabilitation programmes using advanced clinical reasoning and evidence-based practice.
  • Undertake assessments and deliver treatment within community outpatient clinics, patients’ homes, and other community settings.
  • Support early supported discharge pathways following stroke and help prevent unnecessary hospital admissions.
  • Facilitate safe discharge from Hyper Acute Stroke Units (HASUs) and other acute rehabilitation services.
  • Work collaboratively within the multidisciplinary ENABLE Team to deliver integrated rehabilitation services.
  • Monitor patient progress, review rehabilitation goals, and adapt treatment plans to maximise functional recovery and independence.
  • Provide education, advice, and self-management strategies to patients, carers, and families.
  • Maintain accurate clinical documentation in accordance with HCPC standards and Trust policies.
  • Participate in clinical supervision, peer review, reflective practice, continuing professional development (CPD), audit, research, and service improvement activities.
  • Work flexibly to support wider community therapy teams where required to meet service demands.

About You

You will be a HCPC-registered Physiotherapist with experience in neurological rehabilitation, stroke services, or community rehabilitation. You will have excellent clinical reasoning skills and experience managing complex neurological caseloads within multidisciplinary teams.

You will be committed to delivering evidence-based, person-centred rehabilitation and confident working autonomously while collaborating effectively with patients, carers, and healthcare professionals to achieve the best possible outcomes.

Requirements

  • Degree or Diploma in Physiotherapy.
  • Current HCPC registration.
  • Experience working within neurological rehabilitation or stroke services.
  • Experience managing complex neurological caseloads.
  • Knowledge of rehabilitation for conditions including stroke, traumatic brain injury, Parkinson’s disease, Multiple Sclerosis, Motor Neurone Disease, and spinal cord injuries.
  • Strong evidence-based assessment, rehabilitation, and clinical reasoning skills.
  • Experience working within community rehabilitation and multidisciplinary teams.
  • Excellent communication, organisational, and documentation skills.
  • Commitment to continuing professional development, clinical supervision, audit, research, and service improvement.
