Case Worker – Disabled Facilities Grant (DFG)

Brief Job Overview

Tripod Partners are recruiting an experienced Case Worker for a locum opportunity with the London Borough of Newham. Working within East London Social Services, you will join the Home Adaptations to Independent Living (HAIL) Team, providing specialist support in the delivery of major home adaptations through the Disabled Facilities Grant (DFG) process.

Initially offered as a three-month contract, this role is ideal for a highly experienced Social Care Occupational Therapy Assistant or Case Worker with a strong understanding of Disabled Facilities Grant legislation and adaptation processes. You will work closely with Occupational Therapists, residents, contractors, and partner organisations to help people remain safe and independent in their own homes.

Key Responsibilities

  • Manage Disabled Facilities Grant (DFG) cases from Occupational Therapy recommendations through to grant approval and completion.
  • Confirm grant eligibility and assess any client financial contributions in accordance with DFG legislation.
  • Complete and process Disabled Facilities Grant application forms accurately and efficiently.
  • Provide advice and guidance to Occupational Therapists regarding own-scheme adaptations and available funding options.
  • Coordinate equipment adaptations, including stairlifts and ceiling track hoists.
  • Progress Warm and Healthy Homes grants and small repair schemes where appropriate.
  • Work collaboratively with Occupational Therapists, contractors, housing colleagues, and partner organisations to ensure timely delivery of adaptations.
  • Undertake home visits across the borough to support assessments, monitor progress, and liaise with residents.
  • Provide excellent customer service and clear communication throughout the adaptation process.
  • Maintain accurate case records and ensure compliance with legislative and organisational requirements.
  • Contribute to the continued success of a nationally recognised, high-performing Home Adaptations to Independent Living (HAIL) team.

About You

You will have substantial experience working within Adult Social Care, housing adaptations, or Disabled Facilities Grant services. You will possess excellent organisational and customer service skills together with a thorough understanding of the DFG process and relevant housing legislation.

This role would particularly suit an experienced Social Care Occupational Therapy Assistant (OTA) or Case Worker with knowledge of major adaptations, grant administration, and equipment provision.

Requirements

  • Extensive experience working within Disabled Facilities Grant (DFG) services or housing adaptations.
  • Excellent knowledge of Disabled Facilities Grant legislation and grant administration processes.
  • Experience supporting Occupational Therapists with major adaptation schemes.
  • Knowledge of equipment adaptations including stairlifts and ceiling track hoists.
  • Experience progressing housing grants, adaptation projects, or similar case management work.
  • Strong organisational, administrative, and customer service skills.
  • Excellent communication skills and the ability to work collaboratively with multidisciplinary professionals.
  • Ability to undertake home visits across the borough at least two days per week.
  • Suitable for a highly experienced Social Care Occupational Therapy Assistant or equivalent professional.
