  • Complete moving and handling assessments and provide safe handling recommendations for service users and both formal and informal carers.
  • Recommend major and minor adaptations to improve accessibility and support daily living activities within the home.
  • Undertake Disabled Facilities Grant (DFG) assessments and prepare recommendations and specifications for housing adaptations.
  • Complete seating assessments and recommend appropriate seating solutions to maximise posture, comfort, and function.
  • Assess, prescribe, and review specialist equipment and assistive technology to support independent living.
  • Apply the principles of the Mental Capacity Act 2005 when making decisions relating to Occupational Therapy assessments, equipment, and adaptations.
  • Work collaboratively with housing services, health professionals, social care colleagues, and partner agencies to coordinate effective interventions.
  • Maintain accurate case records, assessments, and clinical documentation in accordance with HCPC and local authority standards.
  • Manage an independent community caseload while delivering timely, high-quality Occupational Therapy services.

About You

You will be a HCPC-registered Occupational Therapist with experience working within Adult Social Care or community Occupational Therapy services. You will have strong assessment and clinical reasoning skills, together with experience in moving and handling, equipment provision, housing adaptations, and promoting independence.

You will be confident working autonomously while building effective relationships with service users, carers, housing providers, and multidisciplinary colleagues to achieve positive, person-centred outcomes.

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care or community Occupational Therapy services.
