Brief Job Overview

Tripod Partners are recruiting a motivated and skilled Occupational Therapist for a locum opportunity with Derbyshire County Council. Working within the East Midlands Social Services, you will join the Short Term Assessment and Reablement Team (START), delivering strengths-based interventions that support adults following hospital discharge or during periods of crisis.

This is an excellent opportunity for an Occupational Therapist who is passionate about promoting independence, recovery, and person-centred care. You will play a key role in helping individuals regain confidence, improve functional ability, and reduce long-term care needs through timely assessment, rehabilitation, and collaborative multidisciplinary working.

Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments for adults following hospital discharge or during periods of crisis.
  • Deliver strengths-based, person-centred interventions that promote recovery, independence, and wellbeing.
  • Develop and implement short-term rehabilitation and reablement plans tailored to individual goals and outcomes.
  • Assess functional abilities, mobility, activities of daily living, and environmental factors affecting independence.
  • Recommend and arrange appropriate equipment, assistive technology, and environmental adaptations where required.
  • Support safe and timely hospital discharge, helping to prevent unnecessary admissions and reduce long-term care needs.
  • Work collaboratively with multidisciplinary colleagues, community health teams, social care professionals, and partner agencies.
  • Monitor progress, review interventions, and adapt rehabilitation plans to achieve the best possible outcomes.
  • Maintain accurate clinical records, assessments, and documentation in accordance with HCPC and local authority standards.
  • Contribute to the ongoing development of a high-quality, responsive Short Term Assessment and Reablement Service.

About You

You will be a HCPC-registered Occupational Therapist with experience working within Adult Social Care, community rehabilitation, Intermediate Care, or reablement services. You will be committed to delivering strengths-based practice that enables people to maximise their independence and achieve meaningful outcomes.

You will possess excellent clinical reasoning, communication, organisational, and problem-solving skills, together with the ability to work autonomously while contributing effectively within a multidisciplinary team.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care, community rehabilitation, Intermediate Care, or reablement services.
  • Strong functional assessment, rehabilitation, and discharge planning skills.
  • Experience recommending equipment and promoting independent living.
  • Excellent communication, clinical reasoning, and multidisciplinary team-working skills.
  • Ability to manage a varied community caseload and prioritise workload effectively.
  • Commitment to delivering person-centred, strengths-based Occupational Therapy.
