  • Undertake comprehensive assessments using advanced clinical reasoning to develop person-centred treatment plans.
  • Identify patient goals and deliver rehabilitation programmes that maximise independence and functional ability.
  • Assess and provide appropriate equipment and environmental solutions to support safe discharge and community living.
  • Adopt a holistic approach to rehabilitation, working flexibly across professional boundaries where appropriate.
  • Ensure patients and carers remain at the centre of decision-making throughout assessment and intervention.
  • Work collaboratively with multidisciplinary colleagues to coordinate care and achieve positive patient outcomes.
  • Provide clinical support, education, and supervision to junior staff, Therapy Assistant Practitioners, Integrated Care Workers, students, and non-registered colleagues.
  • Delegate work appropriately while remaining accountable for the quality and delivery of delegated tasks.
  • Prioritise workload according to clinical need and support other locality teams during periods of operational demand.
  • Maintain accurate, timely clinical records in accordance with HCPC and NHS standards.
