Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments for adults with complex care and therapy needs.
  • Develop and implement person-centred intervention plans that maximise independence and wellbeing.
  • Assess functional abilities, mobility, daily living skills, and environmental factors to identify appropriate interventions.
  • Recommend and arrange specialist equipment, assistive technology, and major or minor adaptations where appropriate.
  • Work collaboratively with residents, carers, families, and multidisciplinary professionals to achieve positive outcomes.
  • Support individuals to maintain independence and reduce reliance on long-term care services.
  • Contribute to integrated care planning alongside health, housing, and social care colleagues.
  • Provide professional advice and guidance to service users, carers, and partner organisations.
  • Maintain accurate, timely, and professional clinical records in accordance with HCPC and local authority standards.
  • Promote strengths-based practice and person-centred care throughout all aspects of service delivery.

About You

You will be an HCPC-registered Occupational Therapist with experience working within Adult Social Care or integrated community services. You will have excellent assessment, clinical reasoning, and problem-solving skills, together with the ability to manage a varied caseload independently.

You will be committed to delivering high-quality, person-centred interventions that enable adults with complex needs to remain as independent as possible within their homes and communities.

Requirements
