  • Identify appropriate care pathways and ensure timely access to suitable services and interventions.
  • Act as a specialist resource within Adult Social Care, providing expert advice and contributing to best practice.
  • Work collaboratively within a multidisciplinary team including Occupational Therapists, Sensory Impairment Practitioners, Equipment Services, Social Workers, and health professionals.
  • Support the Team Manager in the day-to-day leadership of the Integrated Care Team and deputise when required.

  • Maintain accurate, timely, and legally compliant case records in accordance with HCPC standards and Adult Social Care legislation.
  • Ensure all interventions reflect strengths-based practice, safeguarding principles, and the values of Haringey Council.
