Brief Job Overview

Tripod Partners are recruiting an experienced Band 6 Specialist Physiotherapist for a locum opportunity within Sussex NHS. Based at Princess Royal Hospital with Sussex Community NHS Foundation Trust, this role is within the Hospital Rapid Discharge Team, supporting patients to leave hospital safely and receive the rehabilitation they need within the community.

This is an excellent opportunity for a Physiotherapist with experience in acute care, rehabilitation, and discharge planning. Working as part of a multidisciplinary team, you will assess patients, facilitate timely hospital discharges, and deliver person-centred interventions that promote recovery, independence, and admission avoidance.

Key Responsibilities

• Provide specialist physiotherapy assessments and treatment for patients within the Hospital Rapid Discharge Team.
• Support safe, timely, and effective hospital discharges through comprehensive rehabilitation planning.
• Assess patients’ mobility, functional ability, and rehabilitation needs.
• Develop and implement personalised rehabilitation programmes to maximise independence.
• Work collaboratively with Occupational Therapists, Nurses, Doctors, Social Workers, and community teams to coordinate discharge plans.
• Provide advice and education to patients, families, and carers to support ongoing recovery.
• Monitor patient progress and adapt treatment plans to meet changing clinical needs.
• Maintain accurate and timely clinical documentation in accordance with HCPC and NHS standards.
• Contribute to effective patient flow, admission avoidance, and high-quality discharge pathways.
• Participate in a seven-day service, working flexibly across the team’s operating hours.

About You

You will be an experienced HCPC-registered Physiotherapist with a strong background in acute care, rehabilitation, or hospital discharge services. You will be confident managing a varied caseload and working effectively within multidisciplinary teams to deliver high-quality, patient-centred care.

You will possess excellent clinical reasoning, communication, and organisational skills, together with the flexibility to support a seven-day service.

Requirements

• Degree or Diploma in Physiotherapy.
• Current HCPC registration.
• Relevant UK NHS Physiotherapy experience.
• Experience within acute hospital, rehabilitation, or discharge services.
• Strong assessment, rehabilitation, and discharge planning skills.
• Excellent communication and multidisciplinary team-working abilities.
• Ability to work independently within a fast-paced clinical environment.
• Commitment to delivering high-quality, patient-centred care.
