Reablement Occupational Therapist

Brief Job Overview

Tripod Partners are recruiting an experienced Reablement Occupational Therapist for a locum opportunity within Berkshire Social Services. Based with Slough Borough Council, this hybrid role offers the opportunity to help adults regain independence and confidence through personalised, strengths-based reablement programmes delivered within the community.

Working as part of a multidisciplinary team, you will support individuals with a wide range of physical, cognitive, and social care needs, helping them achieve meaningful outcomes, reduce reliance on long-term care, and live safely within their own homes. This is an excellent opportunity for an Occupational Therapist who is passionate about recovery-focused practice and service improvement.

Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments within the community using a strengths-based, person-centred approach.
  • Develop, implement, evaluate, and review personalised reablement programmes that maximise independence and wellbeing.
  • Support adults with physical disabilities, mental health needs, learning disabilities, age-related conditions, and other complex needs.
  • Promote functional recovery through evidence-based Occupational Therapy interventions.
  • Facilitate safe and timely hospital discharge while reducing reliance on long-term care wherever appropriate.
  • Recommend ongoing care and support where long-term needs are identified.
  • Assess for equipment, adaptations, and other interventions that support safe independent living.
  • Work collaboratively within a multidisciplinary team alongside health and social care professionals.
  • Support service development and contribute to quality improvement initiatives, assisting managers with delegated projects.
  • Maintain accurate, timely, and professional clinical records in accordance with HCPC and organisational standards.
  • Participate in the team’s rota, including evenings, weekends, and bank holiday working as required.

About You

You will be an experienced HCPC-registered Occupational Therapist with a strong background in community rehabilitation, reablement, or Adult Social Care. You will be passionate about helping people regain independence through strengths-based interventions and confident working autonomously within a multidisciplinary environment.

You will possess excellent assessment, communication, and clinical reasoning skills, together with a commitment to delivering person-centred, recovery-focused care.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care, community rehabilitation, or reablement services.
  • Strong assessment, rehabilitation, and care planning skills.
  • Experience working with adults with physical disabilities, mental health needs, learning disabilities, and older people.
  • Knowledge of strengths-based practice and relevant Adult Social Care legislation.
  • Excellent communication, organisational, and multidisciplinary team-working skills.
  • Ability to work flexibly on a rota, including weekends and bank holidays.
