Occupational Therapist or Physiotherapist – Reablement at Home

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist or Physiotherapist for an exciting locum opportunity within East London Social Services. Based with the London Borough of Hackney, this role is part of an innovative Reablement at Home pilot service designed to help adults regain independence and achieve therapy-led goals within their own homes.

The Reablement at Home service delivers short-term, intensive interventions, typically lasting up to six weeks, enabling people to achieve personalised goals while reducing reliance on long-term care. This pilot will play a key role in shaping the future delivery of homecare services through evidence-based, strengths-focused practice.

Key Responsibilities

  • Undertake comprehensive community-based assessments for adults with permanent, complex, or substantial disabilities.
  • Carry out assessments in accordance with the Care Act 2014 and the Housing Grants, Construction and Regeneration Act.
  • Develop and deliver therapy-led, goal-focused intervention plans that maximise independence and reduce the need for ongoing support.
  • Review existing care packages and identify opportunities to improve independence through rehabilitation and reablement.
  • Assess, prescribe, and review specialist equipment, adaptations, Telecare, and assistive technology solutions.
  • Work collaboratively within a multidisciplinary team to coordinate person-centred care and rehabilitation.
  • Work in partnership with service users, carers, health professionals, housing colleagues, and other stakeholders.
  • Provide supervision and support to Occupational Therapy Assistants and Occupational Therapy students where required.
  • Maintain accurate, timely, and legally compliant case records using relevant recording systems.
  • Contribute to the development of the Occupational Therapy and Sensory Service, supporting wider Adult Social Care objectives.

About You

You will be an experienced Occupational Therapist or Physiotherapist with a strong background in community rehabilitation, reablement, or Adult Social Care. You will be passionate about delivering strengths-based, person-centred interventions that enable people to regain confidence, independence, and control over their daily lives.

You will possess excellent assessment, communication, and clinical reasoning skills, together with the ability to work autonomously while collaborating effectively within multidisciplinary teams.

Requirements

  • Degree or Diploma in Occupational Therapy or Physiotherapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care, community rehabilitation, or reablement services.
  • Knowledge of the Care Act 2014 and the Housing Grants, Construction and Regeneration Act.
  • Experience undertaking strengths-based community assessments.
  • Experience reviewing care packages and supporting rehabilitation-focused interventions.
  • Experience assessing and prescribing equipment, adaptations, Telecare, and assistive technology.
  • Excellent assessment, communication, report-writing, and multidisciplinary team-working skills.
  • Experience supervising assistants or students is desirable.
