Community Occupational Therapist

Brief Job Overview

Tripod Partners are recruiting an experienced Community Occupational Therapist for a locum opportunity within West Yorkshire Social Services. Based with Leeds City Council, this role is within the Recovery Hubs, working alongside multidisciplinary colleagues to deliver rehabilitation, promote independence, and support safe and timely discharge home.

This is an excellent opportunity to join a rehabilitation-focused service that adopts a 24-hour therapeutic approach across health and social care. You will deliver strengths-based Occupational Therapy assessments, coordinate rehabilitation plans, and support adults to achieve their goals while maximising independence within their own homes and communities.

Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments using a strengths-based, person-centred approach.
  • Develop and implement goal-led rehabilitation and outcome-focused intervention plans.
  • Support individuals to maximise independence and safely return to their own homes.
  • Lead and coordinate discharge planning for individuals leaving Recovery Hubs.
  • Work collaboratively with Physiotherapists, Nurses, Support Workers, Social Care colleagues, and wider multidisciplinary teams.
  • Monitor progress and adapt rehabilitation plans to meet changing needs and goals.
  • Undertake moving and handling assessments and implement appropriate risk management strategies.
  • Recommend equipment, adaptations, and interventions that promote safe and independent living.
  • Support adults, children, young people, carers, and families through high-quality Occupational Therapy interventions where appropriate.
  • Maintain accurate, timely, and legally compliant clinical records in accordance with statutory and organisational standards.
  • Contribute to the delivery of high-quality rehabilitation services and continuous service improvement.

About You

You will be an experienced HCPC-registered Occupational Therapist with a strong background in community rehabilitation or Adult Social Care. You will be confident undertaking complex assessments, delivering rehabilitation programmes, and coordinating safe discharge planning within a multidisciplinary environment.

You will possess excellent clinical reasoning, communication, and organisational skills, together with a commitment to strengths-based, person-centred practice that empowers individuals to achieve meaningful outcomes.

Requirements

  • Recognised qualification in Occupational Therapy.
  • Current HCPC registration.
  • Experience working within community rehabilitation or Adult Social Care services.
  • Strong assessment, rehabilitation, and discharge planning experience.
  • Experience undertaking moving and handling assessments and risk management.
  • Ability to develop and implement goal-led intervention plans.
  • Excellent clinical reasoning, communication, and organisational skills.
  • Experience working effectively within multidisciplinary teams.
  • Commitment to strengths-based, person-centred Occupational Therapy practice.
