Tripod Partners are recruiting an experienced Band 6 Occupational Therapist for a locum opportunity within West Yorkshire NHS. Based at Leeds Community Hospital, part of Leeds Community Healthcare NHS Trust, this role offers the opportunity to deliver high-quality rehabilitation services to adults within a community healthcare setting.
The successful candidate will work with adults requiring rehabilitation and discharge support, helping individuals maximise their independence and safely transition from hospital or community services. This is an excellent opportunity for an experienced Occupational Therapist with strong rehabilitation, discharge planning, and multidisciplinary working experience.
Key Responsibilities
- Undertake comprehensive Occupational Therapy assessments for adults requiring rehabilitation and ongoing support.
- Develop and implement person-centred rehabilitation programmes that promote independence and functional recovery.
- Manage a varied clinical caseload, prioritising interventions according to individual needs.
- Lead and contribute to effective discharge planning, ensuring safe and timely transitions of care.
- Assess and implement falls prevention strategies to reduce risk and improve safety.
- Work collaboratively within a multidisciplinary team to coordinate high-quality patient care.
- Provide advice, education, and support to patients, families, and carers.
- Recommend appropriate equipment, adaptations, and community services where required.
- Maintain accurate, timely, and professional clinical documentation in accordance with HCPC and NHS standards.
- Contribute to the delivery of a safe, effective, and patient-centred rehabilitation service.
About You
You will be an experienced Occupational Therapist with a strong background in adult rehabilitation and community healthcare. You will be confident managing your own caseload, coordinating discharge planning, and working effectively within a multidisciplinary team to achieve the best outcomes for patients.
You will possess excellent clinical reasoning, communication, and organisational skills, together with a commitment to delivering evidence-based, person-centred care.
Requirements
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
- Minimum of 2 years’ experience working within adult rehabilitation.
- Experience managing a varied Occupational Therapy caseload.
- Strong discharge planning experience.
- Knowledge and experience of falls prevention interventions.
- Experience working within multidisciplinary teams.
- Good understanding of community care pathways.
- Excellent assessment, communication, and clinical reasoning skills.
