Occupational Therapist – Reablement Team

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity within Wiltshire Social Services. Based at Monkton Park Council Offices, this role sits within the Reablement Team, supporting adults to regain independence and confidence following a hospital stay.

Working within a dedicated multidisciplinary team, you will deliver person-centred, strengths-based Occupational Therapy interventions that help individuals rebuild daily living skills and achieve meaningful outcomes. Key Responsibilities

• Undertake comprehensive Occupational Therapy assessments to identify individuals’ strengths, needs, and rehabilitation goals.
• Develop and implement tailored, person-centred reablement plans that promote independence and recovery.
• Support individuals to regain confidence and functional ability following hospital discharge.
• Use clinical reasoning to assess functional ability and recommend appropriate interventions.
• Provide expert advice on equipment, assistive technology, and safe moving and handling practices.
• Monitor progress and adapt reablement plans to reflect changing needs and abilities.
• Work collaboratively with individuals, carers, health professionals, and multidisciplinary colleagues to achieve positive outcomes.
• Act as a trusted source of professional advice within the Reablement Team, supporting colleagues with clinical decision-making.
• Maintain accurate, timely, and legally compliant clinical records and documentation.
• Contribute to service development, quality improvement initiatives, and the continuous enhancement of reablement services.

About You

You will be an experienced Occupational Therapist with a passion for supporting adults to regain independence through strengths-based, person-centred practice. You will be confident working autonomously while collaborating effectively with colleagues across health and social care.

You will have excellent assessment, communication, and clinical reasoning skills, together with the ability to motivate individuals, provide practical solutions, and support continuous improvement within the service.

Requirements

• Degree or Diploma in Occupational Therapy.
• Current HCPC registration.
• Experience working within Adult Social Care, Community Occupational Therapy, or Reablement Services.
• Knowledge of strengths-based and person-centred practice.
• Experience completing functional assessments and developing rehabilitation or reablement plans.
• Knowledge of equipment provision, assistive technology, and safe moving and handling practices.
• Excellent clinical reasoning, assessment, and report-writing skills.
• Strong communication and multidisciplinary team-working abilities.
• Ability to work independently while supporting colleagues and contributing to service development.
