Reablement Assistant

Tripod Partners are recruiting an experienced Reablement Assistant for a locum opportunity within Hampshire Social Services. Based with Southampton City Council, this community-based role supports adults to regain independence, confidence, and daily living skills following hospital discharge or changes in their health.
Working within a busy multidisciplinary reablement service, you will undertake strengths-based assessments, coordinate short-term reablement plans, and deliver person-centred interventions that promote recovery and reduce the need for long-term care. This is an excellent opportunity for an experienced Senior Carer, Team Leader, or similar professional looking to progress within Adult Social Care.
Key Responsibilities
- Undertake strengths-based assessments and reviews to identify individual needs, goals, and desired outcomes.
- Develop, implement, and coordinate person-centred reablement plans that promote independence and recovery.
- Support individuals following hospital discharge or within the community to maximise functional ability and wellbeing.
- Provide advice, guidance, and signposting to statutory, voluntary, and community services.
- Assess and manage risk while applying Making Safeguarding Personal principles.
- Work collaboratively with Occupational Therapists, Physiotherapists, Social Workers, health professionals, and partner agencies.
- Support effective hospital discharge and service flow, including equipment provision and referral processes.
- Maintain accurate, timely, and legally compliant case records and documentation.
- Provide guidance and role model best practice to frontline colleagues.
- Contribute to service improvement initiatives and support high standards of care delivery.
About You
You will be an experienced care professional with excellent assessment skills and a passion for helping adults regain independence. You will be confident working autonomously, managing a dynamic caseload, and collaborating effectively with multidisciplinary teams to deliver outstanding person-centred care.
You will be proactive, organised, and committed to strengths-based practice, enabling individuals to achieve the best possible outcomes within their own homes and communities.
Requirements
- Minimum Level 3 Diploma/NVQ in Adult Social Care (or equivalent).
- Experience working as a Senior Carer, Team Leader, or equivalent within health or Adult Social Care.
- Demonstrable assessment and care planning experience.
- Experience within reablement, rehabilitation, or short-term intervention services is highly desirable.
- Enhanced DBS (Adult Workforce).
- Strong understanding of strengths-based practice and person-centred care.
- Knowledge of safeguarding principles, including Making Safeguarding Personal.
- Excellent communication, organisational, and problem-solving skills.
- Ability to work independently and collaboratively within multidisciplinary teams.

- Working Hours: 37 hours per week on a flexible rota, including shift working
