Occupational Therapist – Adult Social Care Reablement

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for an exciting locum opportunity within North London Social Services. Based with Enfield Council, this role sits within the Adult Social Care Reablement Service, supporting adults to regain, develop, and maintain their independence following illness, injury, deterioration, or hospital discharge.

Working within a multidisciplinary team, you will deliver high-quality, strengths-based Occupational Therapy assessments and interventions that promote independence, wellbeing, and safe living. Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments within service users’ homes, community settings, and other appropriate environments.
  • Develop, implement, and review person-centred, strengths-based reablement plans that promote independence and wellbeing.
  • Support individuals to regain or maintain skills in activities of daily living, including personal care, mobility, transfers, domestic tasks, meal preparation, and community access.
  • Use advanced clinical reasoning to assess functional ability, environmental barriers, and occupational performance.
  • Recommend, prescribe, and arrange minor equipment, assistive technology, and adaptations to support independent living.
  • Identify the need for major adaptations or longer-term support and make appropriate referrals.
  • Provide specialist moving and handling assessments, risk reduction advice, and guidance on safer techniques for individuals, carers, and colleagues.
  • Contribute to hospital discharge, admission avoidance, and prevention strategies through responsive community-based interventions.
  • Complete Care Act assessments, reviews, and contribute to support planning where ongoing care is required.
  • Undertake Mental Capacity Assessments and Best Interest decision-making in line with the Mental Capacity Act where appropriate.
  • Apply safeguarding principles and respond appropriately to adult safeguarding concerns.
  • Work collaboratively with Occupational Therapists, Physiotherapists, Social Workers, Nurses, GPs, hospital discharge teams, equipment providers, and voluntary organisations.
  • Provide advice, informal training, and professional support to Reablement staff and partner professionals.
  • Maintain accurate, timely, and legally compliant case records and professional documentation.
  • Manage a varied and complex caseload, prioritising work effectively to meet service demands.
  • Participate in supervision, continuing professional development, induction, and service improvement activities.

About You

You will be an experienced Occupational Therapist with a passion for enabling adults to live as independently as possible within their own homes and communities. You will be confident managing complex community caseloads, delivering strengths-based interventions, and working collaboratively across health and social care services.

You will possess excellent assessment, clinical reasoning, and communication skills, together with a strong understanding of Adult Social Care legislation and reablement practice.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Current UK driving licence and access to a car.
  • Experience working within Adult Social Care or Community Occupational Therapy.
  • Strong knowledge of strengths-based practice, reablement, and person-centred care.
  • Experience completing Occupational Therapy assessments, Care Act assessments, and support planning.
  • Knowledge of moving and handling, equipment provision, assistive technology, and housing adaptations.
  • Understanding of the Mental Capacity Act, Best Interest decision-making, and Adult Safeguarding.
  • Excellent multidisciplinary working, communication, and organisational skills.
  • Ability to work independently, including lone working within community settings.
