Senior Occupational Therapy Practitioner – Reablement Front Door

 

Brief Job Overview

 

Tripod Partners are recruiting an experienced Senior Occupational Therapy Practitioner for a locum opportunity within East Sussex Social Services. Based at Woburn, Brighton & Hove City Council, this office-based role will support the delivery of the innovative Reablement Front Door Pilot within Access to Adult Social Care.

 

Working as part of a multidisciplinary team under the guidance of the Lead Occupational Therapist, you will provide strengths-based, person-centred Occupational Therapy interventions at the earliest point of contact. The role focuses on delivering timely screening, triage, and functional assessments that maximise independence, reduce reliance on long-term care, and prevent unnecessary progression onto waiting lists.

Key Responsibilities

 

  • Provide specialist Occupational Therapy input within the Reablement Front Door pilot service.
  • Undertake screening, triage, and functional assessments at the point of referral.
  • Deliver strengths-based and person-centred interventions that promote independence and wellbeing.
  • Identify opportunities for reablement and early intervention to reduce long-term care needs.
  • Support effective flow through Adult Social Care pathways and services.
  • Work collaboratively within a multidisciplinary team to achieve positive outcomes for residents.
  • Provide advice, guidance, and signposting to appropriate services and community resources.
  • Develop enabling intervention plans focused on maximising functional ability and independence.
  • Maintain accurate, timely, and high-quality records in accordance with professional and organisational standards.
  • Contribute to service development, evaluation, and continuous improvement initiatives.
  • Support the implementation of best practice and share learning across the service.
  • Assist with the development of innovative approaches within a pilot and transformation environment.

 

About You

 

You will be an experienced Occupational Therapist with a strong background in reablement, adult social care, hospital discharge, or community-based services. You will be confident undertaking triage, functional assessments, and delivering early intervention strategies that promote independence and prevent escalation of care needs.

 

You will thrive in a fast-paced environment, enjoy collaborative working, and be passionate about supporting service transformation and continuous improvement.

 

Requirements

 

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Significant Occupational Therapy experience within Adult Social Care, Reablement, or Community Services.
  • Strong understanding of strengths-based and person-centred practice.
  • Experience undertaking triage, screening, and functional assessments.
  • Knowledge of reablement principles and early intervention approaches.
  • Experience supporting service development and improvement initiatives.
  • Excellent communication, assessment, and problem-solving skills.
  • Ability to work effectively within a multidisciplinary team environment.
  • Strong recording, case management, and organisational skills.
