Brief Job Overview
Tripod Partners are recruiting a Band 6 Occupational Therapist for a locum role within Sussex NHS. This position is focused on supporting patients within a Frailty Service, providing specialist assessment, rehabilitation, and discharge planning for older adults with complex needs.
The Occupational Therapist will work as part of a multidisciplinary team, supporting frail patients through comprehensive assessment and intervention to maximise independence, improve functional outcomes, and facilitate safe discharge planning.
Key Responsibilities
- Provide specialist occupational therapy assessment and intervention for frail and elderly patients.
- Support rehabilitation and recovery-focused treatment programmes.
- Assess functional abilities and identify barriers to independence.
- Develop and implement person-centred treatment and discharge plans.
- Work collaboratively with multidisciplinary teams to support patient care and discharge pathways.
- Provide advice and support to patients, families, and carers.
- Maintain accurate and timely clinical documentation in line with NHS standards.
- Contribute to positive patient outcomes through evidence-based practice.
About You
You will be an experienced Occupational Therapist with a background in
frailty, elderly care, rehabilitation, or acute hospital services. You will demonstrate
strong clinical reasoning skills and the ability to manage complex patient needs within multidisciplinary environments.
You will be confident working autonomously while delivering high-quality, patient-centred care focused on
maximising independence and supporting safe discharge outcomes.
Requirements
- Recognised qualification in Occupational Therapy.
- Current HCPC registration.
- Experience within frailty, elderly care, rehabilitation, or acute hospital services.
- Strong assessment, rehabilitation, and discharge planning skills.
- Excellent communication and multidisciplinary team working abilities.
- Ability to manage a varied caseload effectively.
