Brief Job Overview

Tripod Partners are recruiting a Senior Occupational Therapist or Physiotherapist – Band 6 for a locum role within Essex NHS. This position sits within the Discharge to Assess (D2A) service, supporting patients within community settings following hospital discharge.

The successful candidate will undertake rapid community-based assessments in patients’ homes and care homes, identifying rehabilitation goals, coordinating care, and supporting safe discharge pathways. The role forms part of a multidisciplinary and multi-skilled team focused on rehabilitation, recovery, and preventing unnecessary hospital admissions.

Key Responsibilities

  • Undertake rapid community assessments for patients on the Discharge to Assess pathway.
  • Assess function, rehabilitation needs, and urgent care requirements within community settings.
  • Develop, implement, and evaluate person-centred rehabilitation goals and treatment plans.
  • Assess equipment needs and coordinate appropriate interventions.
  • Work collaboratively with Primary Health Care Teams, Social Care, Voluntary Services, and CHC teams.
  • Support patients to remain safely at home through coordinated care planning and package setup.
  • Manage complex cases autonomously within professional and service guidelines.
  • Maintain accurate and timely clinical documentation in line with Trust policies.
  • Contribute to supervision, education, and support of junior staff and MDT colleagues.
  • Participate in audit, service improvement, and evidence-based practice initiatives.
  • Undertake training to develop multi-skilled competencies across therapy and healthcare disciplines.

About You

You will be an experienced Occupational Therapist or Physiotherapist with a strong background in community rehabilitation, discharge planning, or urgent care services. You will demonstrate excellent clinical reasoning skills and the ability to work autonomously with complex caseloads.

You will be confident working within fast-paced multidisciplinary environments and committed to delivering person-centred rehabilitation that supports safe discharge and independence.

Requirements

  • Recognised qualification in Occupational Therapy or Physiotherapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience within community rehabilitation or Discharge to Assess services.
  • Experience managing complex caseloads and urgent care assessments.
  • Ability to work autonomously within community settings.
  • Strong communication, organisational, and multidisciplinary working skills.
  • Experience supporting junior staff or contributing to supervision desirable.
  • Commitment to evidence-based practice, audit, and service development.

Contract & Salary
