Brief Job Overview

Tripod Partners are recruiting an Occupational Therapist for a locum role within Central Scotland Social Services. This position is based within a hospital assessment team, with a primary focus on reablement and discharge-to-assess pathways.

The Occupational Therapist will support patients through assessment, rehabilitation, and discharge planning, helping individuals regain independence and transition safely from hospital to community settings.

Key Responsibilities

• Carry out occupational therapy assessments within a hospital setting.
• Support reablement-focused interventions to promote independence.
• Contribute to discharge-to-assess pathways and safe discharge planning.
• Work collaboratively with hospital and community teams to coordinate care.
• Manage a caseload of patients requiring rehabilitation and ongoing support.
• Provide advice and support to patients, carers, and multidisciplinary teams.
• Maintain accurate clinical documentation and case records.

About You

You will be a qualified Occupational Therapist with experience in 
reablement and hospital-based services. You will demonstrate strong clinical reasoning 
skills and the ability to support individuals through rehabilitation and discharge planning.

You will be confident working collaboratively across hospital and community services to deliver 
person-centred outcomes.

Requirements

• Recognised qualification in Occupational Therapy.
• Registration with the Health and Care Professions Council (HCPC).
• Experience within reablement services.
• Experience in hospital or discharge-to-assess pathways desirable.
• Ability to manage a caseload independently.
• Strong communication and organisational skills.
