Brief Job Overview
Tripod Partners are recruiting a Band 6 Occupational Therapist for a locum role within South West Wales NHS. This position is based within an acute hospital setting, supporting patients within frailty services.
The Occupational Therapist will play a key role in delivering frailty assessments, early rehabilitation, and complex discharge planning. The role focuses on supporting patients to regain independence, reduce hospital stay where appropriate, and ensure safe and effective discharge pathways.
Key Responsibilities
- Carry out occupational therapy assessments within acute and frailty settings.
- Deliver early rehabilitation interventions to support recovery and independence.
- Support complex discharge planning from hospital to community settings.
- Manage a caseload of patients with frailty and complex needs.
- Work collaboratively with multidisciplinary teams to coordinate care.
- Provide advice and support to patients, families, and carers.
- Maintain accurate clinical documentation in line with NHS standards.
About You
You will be a qualified Occupational Therapist with experience working within
acute or frailty services. You will demonstrate strong clinical reasoning skills and the
ability to manage complex cases, including discharge planning.
You will be confident working within a multidisciplinary team, supporting patients through rehabilitation
and safe discharge processes.
Requirements
- Recognised qualification in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Experience within acute hospital or frailty services.
- Experience in discharge planning and rehabilitation.
- Ability to manage a caseload independently.
- Strong communication and organisational skills.

- Role: Occupational Therapist – Band 6

- Schedule: Monday to Friday, 08:30 – 16:30 (30-minute break)
