Brief Job Overview

Tripod Partners are recruiting a Band 6 Occupational Therapist for a locum role within East Riding of Yorkshire NHS. This position sits within an Intermediate Care service at a Community Rehabilitation Centre, supporting patients with rehabilitation and discharge planning needs.

The Occupational Therapist will manage a caseload across community rehabilitation settings and patients’ homes, focusing on supporting safe discharge and promoting independence. The role involves working with individuals requiring short-term rehabilitation, including those on Discharge to Assess (D2A) pathways.

Key Responsibilities

• Carry out occupational therapy assessments within intermediate care and community rehabilitation settings.
• Manage a caseload across rehabilitation centres, community settings, and patient homes.
• Support discharge planning, including Discharge to Assess (D2A) pathways.
• Develop and implement treatment plans to promote independence and recovery.
• Work collaboratively with multidisciplinary teams to deliver coordinated care.
• Provide advice and support to patients, families, and carers.
• Maintain accurate clinical documentation in line with NHS standards.
• Travel across sites to support service delivery.

About You

You will be a qualified Occupational Therapist with experience working within 
community rehabilitation or intermediate care services. You will demonstrate strong 
clinical reasoning skills and the ability to support discharge planning and rehabilitation pathways.

You will be confident managing a caseload across multiple settings and working collaboratively within 
multidisciplinary teams to deliver effective patient-centred care.

Requirements

• Recognised qualification in Occupational Therapy.
• Registration with the Health and Care Professions Council (HCPC).
• Relevant UK experience within NHS services.
• Experience in discharge planning and/or Discharge to Assess (D2A) pathways desirable.
• Experience within community rehabilitation or intermediate care settings.
• Ability to manage a caseload independently.
• Full UK driving licence and access to a vehicle (essential).
• Experience using SystmOne desirable.
