Brief Job Overview
Tripod Partners are recruiting an experienced Band 6 Community Occupational Therapist for a locum opportunity within East Riding of Yorkshire NHS. Working across Hull and the East Riding with City Health Care Partnership CIC, this community-based role offers the opportunity to deliver specialist rehabilitation services to adults, with a particular focus on supporting individuals following a stroke.
This is an excellent opportunity for an Occupational Therapist with community stroke rehabilitation experience who enjoys autonomous working while collaborating closely with multidisciplinary colleagues. You will manage your own caseload, helping people maximise their independence and quality of life through evidence-based assessment, rehabilitation, and treatment.
Key Responsibilities
- Provide specialist Occupational Therapy assessments and interventions within community settings.
- Manage a varied caseload of adults requiring rehabilitation, including individuals recovering from stroke.
- Develop and deliver person-centred rehabilitation programmes to maximise independence and functional outcomes.
- Undertake home visits across Hull and the East Riding to assess functional ability and environmental needs.
- Work collaboratively with multidisciplinary teams, GPs, community services, and carers to coordinate high-quality care.
- Recommend appropriate equipment, adaptations, and community support services where required.
- Monitor patient progress, reviewing and adapting treatment plans as needs change.
- Maintain accurate clinical documentation in accordance with HCPC and NHS standards.
- Contribute to safe discharge planning and ongoing community rehabilitation pathways.
- Support the delivery of high-quality, evidence-based Occupational Therapy services.
About You
You will be an experienced HCPC-registered Occupational Therapist with a strong background in community rehabilitation and stroke services. You will be confident working independently, managing your own caseload, and delivering specialist interventions that support adults to remain independent within their own homes.
You will possess excellent clinical reasoning, communication, and organisational skills, together with the flexibility to work across multiple community locations.
Requirements
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
- Relevant UK NHS Occupational Therapy experience.
- Community stroke rehabilitation experience is essential.
- Ideally 3+ years’ post-registration experience.
- Experience managing an independent community caseload.
- Full UK driving licence and access to your own vehicle (essential).
- Experience using SystmOne is desirable but not essential.
- Excellent assessment, rehabilitation, communication, and multidisciplinary working skills.
