Tripod Partners are recruiting a Social Care Assistant for a locum role within
South East London Social Services. This position supports adults and older people with
physical disabilities, mental health needs, and learning disabilities, helping them maintain independence

The Social Care Assistant will undertake strengths-based assessments, develop support plans, and work

Key Responsibilities
- Assess the needs of adults and older people in line with the Care Act and eligibility criteria.
- Develop and implement strengths-based support plans that promote independence and wellbeing.
- Manage a caseload of varying complexity with supervision from senior practitioners.
- Carry out assessments of daily living needs and recommend appropriate solutions such as equipment, adaptations, and assistive technology.
- Undertake risk assessments and support safeguarding processes where required.
- Support service users and carers to access appropriate services, including direct payments and community resources.
- Work collaboratively with multidisciplinary teams and partner agencies to deliver integrated care.
- Conduct reviews of support plans and rehabilitation programmes to ensure effectiveness.
- Maintain accurate and up-to-date records in line with policies and procedures.
- Support urgent visits, welfare checks, and initial safeguarding responses where necessary.
About You
You will have experience working within social care services, supporting individuals with
a range of needs including physical disabilities, learning disabilities, mental health conditions, or
older people. You will be confident undertaking assessments and working in a strengths-based,
person-centred way.
You will demonstrate strong communication and organisational skills, with the ability to manage a caseload
and work collaboratively with colleagues and partner agencies to achieve positive outcomes for service users.
Requirements
- Experience working with individuals with social care needs.
- Understanding of the Care Act and strengths-based practice.
- Ability to undertake assessments, reviews, and support planning.
- Experience supporting moving and handling, equipment provision, or assistive technology desirable.
- Knowledge of safeguarding processes and risk management.
- Strong communication, organisational, and IT skills.
- Ability to work independently and as part of a multidisciplinary team.
