Service: Early Support & Treatment in Early Psychosis

Job Details:

• Band 6 Care Coordinator / Community Psychiatric Nurse required for the Early Intervention in Psychosis service.
• Manage and coordinate a small caseload of service users experiencing early psychosis.
• Carry out assessments of care needs and develop recovery-focused care plans.
• Implement and evaluate evidence-based interventions to support service user recovery.
• Maintain clear, accurate and up-to-date clinical documentation.
• Participate in multidisciplinary team meetings to review cases and allocate care appropriately.
• Undertake health and social care assessments and support ongoing CPA reviews.
• Collect and analyse service data including entry/exit pathways and duration of untreated psychosis.
• Contribute to service quality initiatives including Dialogue and QPR measures.

Skills / Experience:

• Registered Mental Health Nurse / CPN with relevant professional registration.
• Experience working within Early Intervention in Psychosis services (essential).
• Experience care coordinating caseloads within community mental health settings.
• Experience developing and implementing recovery-focused care plans.
• Strong multidisciplinary team working and communication skills.

Working Hours: Part time – 3 days per week (Monday to Friday, 09:00 – 17:00, days to be confirmed).
