Service: Adult Social Care – Adaptations
Job Details:
Occupational Therapist required to support Adult Social Care adaptations service.
Carry out occupational therapy assessments for adults requiring home adaptations.
Assess for and recommend major and minor adaptations to improve accessibility and safety within the home.
Work collaboratively with housing services, surveyors and multidisciplinary teams to implement adaptation solutions.
Support individuals to maximise independence and remain safely within their homes.
Maintain accurate case records and reports in line with service requirements.
Skills / Experience:
Qualified Occupational Therapist with HCPC registration.
Experience working within Adult Social Care.
Experience assessing for housing adaptations.
Knowledge of equipment provision and environmental adaptations.
Ability to manage a caseload independently.
Strong communication and multidisciplinary working skills.
