Job Title: Occupational Therapist or Physiotherapist – Band 6

Location: County Durham and Darlington NHS Foundation Trust – Spectrum / Seaham

Job Details:

  • Band 6 Occupational Therapist or Physiotherapist required to start ASAP within community therapy services.
  • Provide specialist assessments and rehabilitation interventions for adults with physical dysfunction or disabilities.
  • Manage a varied caseload including patients with complex needs across a range of community settings.
  • Work within crisis response services, intermediate care beds, rehabilitation wards, patients’ homes and care homes.
  • Develop and implement individualised rehabilitation and treatment programmes.
  • Support discharge planning from Intermediate Care beds.
  • Assess and prescribe equipment, including manual handling equipment.
  • Work closely with the multidisciplinary team including consultants, GPs, nurses, social workers and other allied health professionals.
  • Provide supervision and support to junior staff and therapy assistants.
  • Contribute to clinical education and training across the community service.

Skills / Experience:

  • HCPC registered Occupational Therapist or Physiotherapist.
  • Experience in community therapy, rehabilitation or reablement services.
  • Experience planning treatment programmes and managing discharges from Intermediate Care.
  • Manual handling experience and equipment provision skills.
  • Knowledge of common conditions such as arthritis, heart disease, diabetes, dementia, MSK conditions, COPD and falls.
  • Experience supervising junior staff.
  • Full UK driving licence and access to a vehicle (essential).

Working Hours: 37.5 hours per week, 08:30 – 16:30.
