Job Details:

 

  • Band 6 or above Occupational Therapist or Physiotherapist required to start ASAP within community therapy services.
  • Manage an independent caseload including patients with complex needs across a range of community environments.
  • Work across community settings including crisis response services, intermediate care beds, rehabilitation units, patients’ homes and care homes.
  • Undertake specialist assessments and develop individualised treatment and rehabilitation programmes.
  • Support discharge planning and rehabilitation for patients transitioning from Intermediate Care beds.
  • Assess, recommend and provide equipment, including manual handling equipment.
  • Work collaboratively with the multidisciplinary team including consultants, GPs, nurses, social workers and allied health professionals.
  • Supervise and support junior staff and therapy assistants.
  • Deliver clinical education and training for staff within community services.

 

Skills / Experience:

 

  • HCPC registered Occupational Therapist or Physiotherapist.
  • Experience in community work, rehabilitation or reablement services.
  • Experience developing treatment plans and supporting discharge from Intermediate Care.
  • Manual handling experience and equipment provision skills.
  • Knowledge of conditions such as arthritis, heart disease, diabetes, dementia, MSK conditions, COPD and falls.
  • Experience supervising junior staff.
  • Full UK driving licence and access to a vehicle (essential).

 

Working Hours: 37.5 hours per week, 08:30 – 16:30.
